Guía para la vigilancia del coronavirus novel 2019(2019-nCoV)
Este documento presenta una guía preliminar para acción de vigilancia temprana, detección y el
reporte inmediato de un caso con sospecha a el coronavirus novel 2019(2019-nCoV). El virus
parece ser una cepa de coronavirus nunca antes vista, una gran familia de virus que puede causar
enfermedades que van desde el resfriado común hasta el síndrome respiratorio agudo severo
(SRAS).
Un brote de pulmonía de etiología desconocida en Wuhan, China, fue inicialmente reportado en
diciembre 31, 2019. Actualmente se ha extendido a múltiples países, incluyendo Estados
Unidos, y no existen casos reportados en Puerto Rico. Los coronavirus que causan enfermedad
en humanos son causantes de infección en las vías respiratorias superiores; produciendo síntomas
como:
secreciones nasales
congestión nasal
dificultad respiratoria
dolor de garganta
estornudos
tos
fiebre
I. ¿Cómo se transmite?
En un principio se estableció que la transmisión del virus solo podía pasar de animales a
humanos, igual que otros patógenos conocido de la misma familia: el MERS-CoV; pero se ha
confirmado que puede contagiarse de persona a persona, principalmente por vía respiratoria.

II. El criterio para la evaluación de pacientes bajo investigación del 2019-nCoV es el
siguiente:
Síntomas Clínicos & Riesgo Epidemiológico
Fiebre1 o síntomas de enfermedad de las
vías respiratorias bajas (ej., tos o
dificultad para respirar) Y
Cualquier persona, incluyendo los proveedores de
servicios de salud, que haya tenido contacto cercano
con un paciente hospitalizado con 2019 nCoV, con
un laboratorio, en los pasados 14 días anterior al
inicio de los síntomas
Fiebre1 y síntomas de una enfermedad
de respiratoria bajas (ej., tos o dificultad
para respirar) Y
Un historial de viaje desde la provincia de Hubei,
China, en los pasados 14 días anterior al inicio de
los síntomas
Fiebre1 y síntomas de una enfermedad
de las vías (ej., tos o dificultad para

pasados 14 días anterior al inicio de los síntomas
Refiérase al Anejo V para evaluación de pacientes que puedan estar enfermos o que hayan estado
expuestos al nuevo coronavirus 2019 (2019-nCoV): “Flowchart to Identify and Assess 2019 Novel
Coronavirus”.
Nota: 1Fiebre puede no estar presente en algunos pacientes (ejemplo: jóvenes, mayores de edad,
inmunosuprimidos, o tomando medicamentos para bajar fiebre.)
2Contacto cercano definido como:
a) Estar dentro de un radio de 6 pies o dentro del cuarto de un caso de 2019-nCoV por un
período prolongado de tiempo, sin utilizar equipo personal protectivo recomendado.
b) Tener contacto directo con secreciones infecciosas de un caso de 2019-nCoV, sin utilizar
equipo personal protectivo recomendado.

III. Notificación del caso con sospecha a 2019-nCoV al Departamento de Salud
Todo proveedor de salud y/o personal de Control de Infecciones, deberá notificar
inmediatamente vía telefónica (categoría III) al Departamento de Salud: Programa de
Epidemiología (Nivel Central/Regional), la existencia de un paciente bajo investigación para
2019-nCoV. A su vez, esta notificación se hará mediante la utilización de la hoja de Categoría I
y la hoja de investigación de caso: Interim 2019 novel coronavirus (2019-nCoV) patient under
investigation (PUI) form; cual es requerida por CDC, para realizar la prueba diagnóstica de
2019-nCoV (rRT-PCR panel). La notificación del historial de viaje debe ser detallada.
IV. Recomendación para colección de muestras
Una vez el caso haya sido notificado al Programa de Epidemiología; el
CDC’s Emergency Operation Center (EOC), colaborara con el
Departamento de Salud (Laboratorio) para la colección, almacenamiento y
envío de muestras al CDC.
La prueba diagnóstica para 2019-nCoV debe ser realizada por el CDC.
Por razones de bioseguridad, no es recomendable realizar aislamiento de
virus en cultivo celular o iniciar caracterización de agentes virales
recuperados en cultivos de muestras en pacientes bajo investigación (PIU)
para 2019-nCoV.
CDC recomienda obtener múltiples muestras de laboratorio, incluyendo
secreciones del tracto respiratorio, suero, y considerar otros tipos de
muestras como excreta y orina.
El envío de muestras debe ser coordinado con el Laboratorio del
Departamento de Salud, y debe someterse con la Hoja reporte 50.34
(CDC Specimen Submission Form) y la hoja de caso bajo investigación
(PUI): Interim 2019 novel coronavirus (2019-nCoV) patient under
investigation (PUI) form.
