Que es GeneXpert ?

completed at least 12 months post-treatment follow-up.
- The strongest risk factor for a bacteriologically unfavorable outcome was high-level FQ
resistance, particularly when compounded by initial pyrazinamide (PZA) resist.
- Low-level FQ resistance had no unfavorable effect on treatment outcome.
- 12-month standardised regimen: 4 Gx-Cfz-E-Z-Kn-Pth-H / 8 Gx-Cfz-E-Z
- 65 MDR-TB were included and analysed: Cure: 58 patients (89.2%), 6 died and 1
defaulted.
- All 49 patients assessed at the 24- month follow-up after cure remained smear-
and culture-negative.
- One of 58 patients tested for HIV (1.7%) infection was positive.
- Twenty-five patients (39.7%) were severely affected (BMI 616 kg/m2).
- The main adverse events were vomiting (26.2%) and hearing impairment (20%),
but no treatment had to be stopped.
- Standardised 12-month regimen including gatifloxacin, clofazimine,
prothionamide, ethambutol and pyrazinamide throughout, supplem. by kanamycin
and isoniazid during an IP of a minimum of 4 months.
- Among the 150/236 eligible and treated patients:
- 134 (89%) successfully completed treatment, 10 died, 5 were lost, 1 failed
and none relapsed.
- 30 (20%) were HIV positive and 41 (27%) had radiographic involvement of five or
six of the six lung zones.
- The most important adverse drug event was hearing impairment (43%).

- 41 XDR-TB with response to any available chemotherapeutic option
during the previous 6 months � Randomly assigned to linezolidtherapy that started immediately or after 2 months, at a dose of 600
mg per day, without a change in their background regimen.
- By 4 months, 15 of the 19 patients (79%) in the immediate-startgroup and 7 of the 20 (35%) in the delayed-start group had culture conversion (P=0.001).
- Most patients (34 of 39 [87%]) had a negative sputum culture within
6 months after linezolid had been added to their drug regimen.

- The proportion of sputum culture conversions in the linezolid group was
78.8% by 24 months, significantly higher than that in the control group
(37.6%, p,0.001).
- The treatment success rate in linezolid group was 69.7%, significantly
higher than that in the control group (34.4%, p50.004).
- 27 (81.8%) patients had clinically significant adverse events in the linezolid
group, of whom 25 (93%) patients had events that were possibly or
probably related to linezolid. Most adverse events resolved after reducing
the dosage or temporarily discontinuing Lz.

- 29 MDR-TB treatment failure patients (sixteen had laboratory proven XDR-TB, and rest 13 had MDR-TB with resistance to any quinolone but sensitive to injectables)
- All patients received daily unsupervised therapy with linezolid, one injectable agent, one fluoroquinolone and two or more other drugs.
- Out of total 29 patients, 89.7% patients achieved sputum smear and culture conversion; 72.4% showed interim favorable outcome; 10.3% died, 6.8% failed and 10.3% patients defaulted.
- Linezolid had to be stopped in 3 (10.3%) patients due to adverse reactions.
- The outcome of treatment of 16 XDR-TB patients was comparable to the other 13.

- Single daily administration of 375 mg (2) LZ led to a similar maximum
concentrations (Cmax) in serum (10.8 mg·L−−−−1), which is exceeding the
predicted MICs twenty-fold [2].
- KOH et al. [3] could even show that a mean Cmax of 11.6 mg·L−−−−1 (range
1.5–15 mg·L−−−−1) could be achieved by a daily use of 300 mg.

- TMC207 + régimen de 2da línea vs placebo + régimen de 2da línea por 8
semanas, esterilizó el esputo en 48% de los pacientes vs 9% en el caso
del placebo.
- Al cabo de 2 años de tratamiento, se curaron 81% de los pacientes que
recibieron TMC207 + régimen estándar vs 57% de los que recibieron solo
el régimen estandar
- On the basis of WHO outcome definitions for MDR-TB, cure
rates at 120 weeks were 58% in the bedaquiline group and 32%
in the placebo group (P=0.003).
- The overall incidence of adverse events was similar in the two
groups ���� There were 10 deaths in the bedaquiline group and 2
in the placebo group, with no causal pattern evident.
- Laboratory testing of the minimal inhibitory concentration (MIC)
of bedaquiline seems to suggest a breakpoint for susceptibility at
<0.5μμμμg/ml in agar medium; however, until a specific DST assay
for bedaquiline is developed, clinicians will not be able to be
guided by MIC values or DST results when composing a regimen
- Dose: 400mg daily for the first two weeks, followed by 200mg
three times per week at least 48 hours apart, for a total maximum
duration of 24 weeks.
- Available data suggest better uptake of bedaquiline when
administered with food.

- The mean 14-day EBA of PA-824-moxifloxacin-pyrazinamide (n=13; 0·233 [SD 0·128]) was significantly higher than that of bedaquiline (14; 0·061 [0·068]), bedaquiline- pyrazinamide (15; 0·131 [0·102]), bedaquiline-PA-824 (14; 0·114 [0·050]), but not PA-824-pyrazinamide (14; 0·154 [0·040]), and comparable with that of standard treatment (ten; 0·140 [0·094]).
- Treatments were well tolerated and appeared safe. One patient on PA-824-moxifloxacin-pyrazinamide was withdrawn because of corrected QT interval changes exceeding criteria prespecified in the protocol.
- Interpretation: PA-824-moxifloxacin-pyrazinamide is potentially suitable for treating drug-sensitive and multidrugresistant tuberculosis.

combinations with Pa and B.
- The primary endpoint was the mean daily fall in log10 M. TB colony
- Estimated activities were:
- 0.167 (95% CI: 0.075 - 0.257) for B-Pa-Z- 0.151 (95% CI: 0.071 - 0.232) for standard treatment
- 0.124 (95% CI: 0.035 - 0.214) for B-Z-C
- 0.115 (95% CI: 0.039 - 0.189) for B-Pa-Z-C
- 0.076 (95% CI: 0.005 - 0.145) for B-Pa-C
- Z alone had modest activity (0.036; 95% CI: -0.026 - 0.099)
- C had no activity alone (-0.017; 95% CI: -0.085 - 0.053) or in combinations.
- B-Pa-Z, including two novel agents without resistance in prevalent M. TB
strains, is a potential new tuberculosis treatment regimen.
- Delamanid (OPC-67683) is a nitro-dihydro-imidazooxazole derivative that
inhibits mycolic acid synthesis.
- In a recently published randomized placebo-controlled multinational
clinical trial, 481 patients were assigned to receive delamanid 100 mg twice
daily, delamanid 200 mg twice daily, or placebo for 2 months in
combination with an optimized background regimen.
- Culture conversion at 2 months in a liquid culture system was more likely in
patients who received delamanid 100 mg twice daily (45.4%, p = 0.0008))
or delaminid 200 mg twice daily (41.9%, p=0.04) than placebo.
- Similarly with solid media, conversion occurred in 53.8%, 65.2% and 33.6%,
respectively.
- Adverse events were distributed relatively equally across the three groups
except that QT prolongation was more common in the delamanid group.

- Favourable outcomes were observed in 143/192 patients (74.5%) who
received delamanid ≥≥≥≥6 months, compared to 126/229 patients (55.0%)
who received delamanid for ≤≤≤≤2 months.
- Mortality was reduced to 1.0% among those receiving long-term delamanid,
versus short-term/no delamanid (8.3%), p<0.001.
- Treatment benefit was also seen among patients with XDR-TB
- Conclusion: This analysis suggests that treatment with delamanid for 6 months
in combination with an optimized background regimen can improve
outcomes and reduce mortality among M/XDR-TB
- Dose in adults is 100mg twice a day, irrespective
of body-weight, for a period of six months.
- As bioavailability was higher when given after a standard meal,
delamanid should preferably be delivered after a meal.
- There was no evidence that delamanid 200mg twice a day was
more effective than the 100mg dose and the higher dose was
associated with higher rates of adverse events including QT
interval prolongation
- There is as yet no standardized DST method for delamanid, nor a
commercially available test
