INSTRUIR, ENSEÑAR Y EDUCAR
n Colombia, que lleva casi 20 años un sistema salud que ha alcanzado cerca
del 100% en cobertura en salud a los colombianos, se discute una
modificación estructural de esa ley, la cual es conocida como Ley 100 de
1993. La nueva reforma, pretende corregir sus defectos: la intermediación, la
integración vertical y el modelo de salud que ha estado centrado en la atención de la
enfermedad. Quedó en el olvido la prevención y sus medidas .Estas - las más eficaces-
cuando miramos los resultados en costo efectividad.
El Gobierno Nacional, bien intencionado, pero errado, buscando facilitar y agilizar la
oportunidad para la atención de los especialistas, ha propuesto en el nuevo proyecto de
Ley que hospitales con “experiencia” y sin control universitario, puedan otorgar títulos
de Especialistas a Médicos que durante un periodo de tiempo hayan trabajado como
hospitalarios en esa institución. Así, se aumentan en un periodo corto el número de
”especialistas” y se satisface la demanda.
¿Estamos preparados en Latinoamérica para que hospitales de tercer nivel, sin aval
universitario, otorguen Título de Neurocirujano a Médicos hospitalarios que durante 5
años han trabajado como “médicos de planta” en un servicio de neurocirugía?
Más allá de la pregunta planteada, llegamos al punto de la oferta y en esta la
distribución y concentración de los especialistas en nuestros países. Tenemos casi en
todos los países latinos 1 neurocirujano por 100.000 habitantes y esta cifra se aproxima
a los estándares de la distribución adecuada que la WFNS sugiere. Pero si detallamos
esta distribución, vemos que en las grandes ciudades se concentran y alcanzan casi
cifras del 20/100.000. Los especialistas que se radican en las grandes ciudades
encuentran oportunidades de trabajo asistencial y remuneración que apenas alcanza
para los gastos esenciales de una familia. El sueño de hacer los grandes casos no existe,
la ilusión del consultorio es una quimera y el horario de las grandes ciudades, donde el
día tiene 18 horas (transporte, movilización,), solo permite trabajos en dos instituciones
con cargas laborales de 8 horas que copan nuestra capacidad física.

La carga asistencial los asfixia; cuando desean asistir a Jornadas de Actualización la
responsabilidad laboral no se los permite y la fatiga del turno de la noche anterior los
obnubila.
Estos especialistas atiborrados en grandes capitales se frustran; nada más peligroso que
especialistas fracasados, amargados y desconsolados que se han movilizado
descrestados por las “luces académicas” que aparentemente ofrecen las grandes
ciudades.
La primera tarea como institución continental es facilitar la distribución de los
especialistas en neurocirugía para que la densidad de nuestra especialidad no tiña
solamente la ubicación geográfica de las grandes ciudades.
Debemos llegar a las ciudades intermedias y como centro de pensamiento recomendar
a nuestros gobiernos actualizar los recursos físicos para que el Medico Neurocirujano en
esa región pueda solucionar el 90% de la patología que aqueja a esa comunidad. Esta es
la neurocirugía general y básica.
Así, podemos atender desde el tumor supratentorial, el aneurisma simple y la estenosis
cervical. El tumor petroclival, el aneurisma de la basilar y la escoliosis compleja deben
ser remitidos a centros altamente calificados en las ciudades, donde la experticia se
nutre de la frecuencia y los resultados de calidad compensan los altos costos.
¿Cuál es el médico neurocirujano que necesita Latinoamérica?
Detengámonos en el currículo; tengo la convicción que no hay un centro en América
Latina que reúna las condiciones ideales para formar el neurocirujano integral.
Necesitamos abolir las fronteras y fomentar los currículos flexibles para que el
neurocirujano en formación se beneficie de las grandes oportunidades que nuestro
continente ofrece.
Hay centros en todos los países fuertes en áreas específicas y si nosotros en un gesto de
madurez (dejemos ese prurito latino tonto: “me las se todas”) reconocemos sus
bondades, podemos ofrecer a nuestros residentes y neurocirujanos este escenario
autóctono de aprendizaje. Estos compensan el anhelo escondido de capacitación y
prevenimos así esa migración triste a las grandes ciudades donde la frustración empaña
la belleza del ejercicio de una elite privilegiada, como los neurocirujanos de un
continente, donde el 50% de sus habitantes están clasificados como pobres.
La pobreza no son los recursos materiales; la peor de las pobrezas es la indigencia
intelectual: la ignorancia.
Afligido vive el país con “pobres de bolsillo”; sin futuro la nación con “pobres de mente”
y sin sueños la patria con “pobres de corazón”.

Aquí es donde la FLANC y sus sociedades federadas tienen que jugar el partido de su
vida. Estimular los procesos de capacitación independientemente de quién los
promueva, fomentar el desarrollo de más y mejores escenarios de formación. Apoyar
liderazgos proactivos y ayudar a los servicios débiles para que crezcan sanos y se
robustezcan. Llevar las experiencias exitosas para quienes se inician en estas actividades
no repitan los mismos errores y sobretodo tener como punto diana el neurocirujano de
provincia para que no se sienta distante de sus pares y pueda desde las regiones,
convertirse en agente de cambio que promueva la salud integral de sus conciudadanos.
Hacer fuerte a los débiles; debe ser el primer postulado en nuestros reglamentos y
convicciones.

La FLANC en el Contexto Mundial.
n el inicio del siglo XX las ciencias en general inician un crecimiento exponencial
en áreas como la Anatomía, la Fisiología y la Patología. Esto aunado al interés,
perseverancia y afán investigativo de muchos hombres, promueve el desarrollo
de muchas áreas como las Neurociencias. Indiscutiblemente el desarrollo
tecnológico que acompaña a esta época permitió que los limitados recursos fuesen cada
vez un menor impedimento para el desarrollo y crecimiento.
A nivel mundial hombres como Cushing, Egaz Moniz, Olivecrona, Asenjo y una selecta
lista de precursores de la Cirugía Neurológica abrieron las puertas para que se sentarán
las bases de lo que hoy conocemos como la Neurocirugía Moderna, la cual continúa
teniendo sus bases en aquellas investigaciones iniciales de la Anatomía, Fisiología y
Patología.
La epidemiologia de la patología Neuroquirúrgica ha ido cambiando de acuerdo a
diferentes variables, dentro de las cuales el desarrollo no solo tecnológico sino también
económico social que cada una de las regiones ha presentado ha influido definitivamente
en estos cambios. Es indiscutible que en países desarrollados como Estados Unidos,
Alemania, Japón, Francia o Inglaterra entre otros, las investigaciones y recursos
tecnológicos se orienten en primera fila a la patología oncológica o cerebro vascular y es
muy cierto que al igual que ellos en muchos países de América Latina dicha patología
ocupa una ubicación entre sus primeras causas de muerte tal como sucede en países
como Brasil, Argentina, México, Costa Rica o Cuba entre otras, y si revisamos en las
estadísticas nacionales de países que se encuentran en "vías de desarrollo" como la
región Centroamericana, encontraremos que también ocupan un sitial muy importante
dentro de sus estadísticas de morbimortalidad. Por eso es muy importante conocer el
aporte que estas Naciones nos ofrecen y poderlo brindar a nuestros pacientes. De allí la
importancia del intercambio de conocimientos y experiencias que la FLANC ofrece a
través de sus Congresos Latinoamericanos, cursos, becas a Neurocirujanos jóvenes o

Residentes de Neurocirugía. La Revista de la FLANC es probablemente uno de los
elementos más valiosos con los que contamos para lograr ese intercambio regional de
experiencias que enriquecen nuestro Servicio a los Enfermos. La creación de los diferentes
Capítulos Académicos es sin duda una de las ideas de mayor valor, ya que de una forma
más definida se tiene la oportunidad de abordar diferentes temáticas, ya sea espinal,
trauma, Oncología, vascular entre otras.
Uno de los Capítulos que en mi opinión alcanza una relevancia muy especial en América
Latina, es el Capítulo de Neurocirugía Pediátrica.
Al revisar la Neuro epidemiologia Pediátrica, es muy fácil de apreciar la enorme brecha
existente (al hablar de patología congénita) entre los países desarrollados y los llamados
en vías de desarrollo. La presencia de una enorme estadística en patologías que se
acompañan de disrafismo espinal y/o hidrocefalia en países del África o de América
Latina al compararla con la de los países desarrollados, nos muestra (entre otras cosas)
del enorme problema socio económico que invade estas regiones y que acompañan a
nuestras poblaciones.
Hablar de Investigación en América Latina resulta usualmente muy poco alentador. Sin
embargo uno de los objetivos de la FLANC es promoverla y lograr a través de ella
propuestas que incidan en el bienestar de aquellos a quienes nos debemos.
Afortunadamente tenemos una organización académica activa y pujante. Cooperemos
con la FLANC para que la investigación, propuestas y ejecución de las mismas se hagan
realidad. América Latina tiene frente a sí, un enorme reto. Enfrentémoslo.

Tratamiento de la Columna Toracolumbar Fracturas y Dislocaciones y su Influencia en el Proceso de Rehabilitación Treatment of Thoracolumbar Spine Fractures and Dislocations Its Influence on the Rehabilitation Process Marcos Masini, MD, M. ABSTRACT
The author conducts a retrospective analysis of 121 patients suffering from fracture
and/or dislocation of the thoracolumbar spine treated within the protocol and
undergoing surgery between 1988 and 1995, with long outpatient monitoring
treatment.
An operative protocol and subsequent rehabilitation program were utilized in all
patients. Ninety-five patients were men, in their third and fourth decades of life. Fifty-
seven patients presented injuries caused by traffic accidents: forty-one by falls and
twenty-three by local traumas. Eighty-eight bone injuries were located at the T11, T12
and L1 transition and 75 patients presented paraplegia upon hospital admission. All of
them were classified by Frankel/ASIA scale at admission, discharge and last outpatient
follow up. All patients underwent plain x-rays of the spine. Seventy-one examinations
were complemented with computerized tomography and 50 with magnetic resonance.
All of them received rectangle rods and segmental sublaminar wire fixation with bone
graft fusion. Eighty-five patients needed concomitant decompression of the spinal
canal due to associated neurological deficit.
Patients admitted with a complete neurological deficit had no improvement during
follow up, however 70 percent of patients with partial neurological injuries improved
at least one Frankel/ASIA grade by the end of follow up. Lumbar injuries improved the
neurological grade 5 times more than localized thoracic injuries. Antero-posterior
dislocations of the vertebral body were corrected in average from 37 to 16 percent,
lateral dislocations from 27 to 19 percent and anterior angulation from 21 to 12
degrees. The loss due to angulation in the 43 patients examined after discharge did not
affect the functional independence. Patients’ functional independence measure
improved from an average of 29 points at the admission to an average of 79 points
after about 60 days in rehabilitation. Functional rehabilitation results in final
measurement were poor, however, when related with brain injury. The shorter the

time between the injury and admission, the shorter the hospital stay, rehabilitation
period, and the better the final independence measure and hospital efficiency ratio as
well. The timing between injury and the surgical procedure did not influence the final
neurological recovery of patients and those patients having immediate post injury
surgery, however had better rehabilitation grade, i.e., the shorter the timing between
injury and surgical procedure, the shorter the hospital stay. The timing between the
injury and the surgical procedure also influences the efficiency ratio, i.e., the shorter
the timing between injury and surgery, the better the evaluation of the hospital
efficiency. Also, the shorter the timing between the injury and the patient admission,
the shorter the hospital stay, the better the evaluation in the FIM scale and the better
the Efficiency Ratio. The time patients waited for surgery, already at the hospital, also
did not increase the stay. The only factor delaying spine surgery was concurrent with
patients presenting thoracic, abdominal and brain injury. Burst fractures, differently
from dislocation, were found to be prone to late re-angulation due to impairment in
the anterior support of the spine. However, this did not influence the final functional
independence measure and did not cause neurological deficit.
The surgical technique used to treat patients with fractures and/or dislocation of
thoracolumbar spine is efficient. Although most thoracolumbar spine injuries are
statistically treated as non–surgical, the timing to conduct surgery must consider the
total clinical condition of the patient at admission.
Thus, the surgical procedure is elective. This method aims for concomitant spinal cord
and root decompression and spinal arthrodesis fixation of the spine. Decompression of
spinal cord and root should be always considered when deficit and misalignment on
the spine canal is recognized. The spinal stabilization is immediate and there is no need
for postoperative external immobilization. This method alleviates the pain and
stabilizes the deformity, enabling to continue the rehabilitation program.
Complications are similar to the ones of other methods and are treatable. The safety,
simplicity and economy of this system justify it as an option in this treatment.

INTRODUCTION
The spinal cord injury is one of the most devastating diseases for individuals of
modern society, affecting mainly young healthy and economically active people,
causing great economic and social impact¹. Estimates point out to more than 10,000
cases of spinal cord injury per year in Brazil. Trauma is the predominant cause. The

numbers are alarming, exceeding most of the statistics published relating to the
incidence of spinal cord injuries in several countries. The United States and Japan, for
example, show rates of 40 new cases per every million of inhabitants per year. In
Brazil, the average rate in 1997, was 71 new cases per million of inhabitants.
Southwest and Northeast regions showed, respectively, rates of 71 and 91 new cases
per million. Half of the patients suffer from thoracolumbar spinal injuries 2, 3.This is a
problem that grows with Brazil, requiring much attention in addition to education and
prevention campaigns, as well as more efficient rehabilitation of the surviving
population, currently calculated at more than 180 thousand individuals.
In recent decades, there has been great progress in treatment practices for patients
with spinal cord injuries, and clinical evaluation has been systematized. Radiological
diagnosis methods have improved and the knowledge of spinal biomechanics has
enabled better identification of instability. Rescuing methods have been improved as
well as the clinical treatment of spinal cord injuries themselves and their
complications. Several fixation systems for the unstable spine have been developed
and the rehabilitation process has been systematized. Prevention programs have been
focused on causal factors and even the profile of spinal cord injuries has changed in
recent years, with the increase in spinal injuries due to aggression and the higher
incidence in the elderly population.
The thoracolumbar spine injuries have been the theme for a number of publications,
however, there were only a few prospective protocol works with sufficient amounts of
cases and monitored long enough to enable a statistical analysis with the use of
Methylprednisolone (NASCIS 1, 2 and 3), Ganglioside (SYGEN), Fampridine (SR) and
Cellular Therapy (Macrofage Trial)4. Thus, almost all aspects of treatment are still
surrounded with controversy. Knowledge of the problem and its dimensions has
become important to the surgeon once these patients require a multidisciplinary
approach, specialized care and rehabilitation.
Discussing technicalities such as fixation and arthrodesis of the thoracolumbar spine is
a detail that encourages a deeper investigation of the magnitude of the issue. The
monitoring of patients over time may enable a new clinical and psychological approach
and find new possibilities and solutions.
Our study on the topic began in January 1980. In the first 16 years of that period, we
treated 1,090 patients with thoracolumbar spine fractures and dislocations.
Considering those patients, 605 (55.5 percent) presented neurological deficit, 308 (28
percent) underwent surgery and 224 (73 percent) of them were instrumented by
posterior approach. In temporal sequence, 53 (24 percent) were treated with
Harrington instrumentation, 50 (22 percent) with Harrington-Luque instrumentation
and 121 (54 percent) with rectangular instrumentation. The last 121 patients are the

focus of our study. The Pedicle Fixation system with Rods and their variants have been
used in patients after the period of time of this study.
PATIENTS AND METHOD
This study includes 121 patients
suffering from thoracolumbar spine
fractures and dislocations who were
surgically treated (DOVE)64, analyzed
in 100 different items and monitored
for an average period of five (5) years
after the surgical procedure. This is a
protocol study where the identification
of the 121 patients was conducted
through hospital record. The analysis
period was from January 1988 to
December 1995. All patients, with
thoracic and lumbar spine injuries,
most of them concentrated between T-
11 and L-3, such region defined as
thoracolumbar transition. Based on the
information collected from the
admission protocol and the medical
records, the following patient data
were analyzed: gender, age, origin,
cause of injury, related injuries, dates
of fracture, hospital admission, surgery,
discharge, and date of the last
appointment at the hospital (base of
analysis: December 1996), neurologic
deficit on hospital admission, discharge
and follow up appointment, grade of
injury, grade fixed, number of grades
fixed, rod type, size of the rod used,
blood loss, surgery time, use or non-use
of blood or derivatives, procedure
details, date of deambulation after the
procedure (Frankel/ASIA - D and E)5,7
and date of rehabilitation
commencement (Frankel/ASIA-A, B and
C)5,7, neurological level at hospital
admission and discharge and number of
follow-up days after discharge.
Other records or procedures conducted
included: complications before and
after the surgical procedure at the
rehabilitation hospital and later
occurrences after hospital discharge,
classification on the FIM (functional
independence measure of the patient)
scale on admission and discharge, date
of the admission radiological study,
postoperative control and late control
conducted at discharge or in
ambulatory at the time of the last
appointment. During radiological
studies the following aspects were
analyzed: angulation, anteroposterior
and laterolateral traslation, scoliosis;
date of the computerized tomography
and magnetic resonance, with analysis
of the degree of commitment of the
spinal canal, classification of fractures
and dislocations by the MARGERL and
GERTZBEIN (6) system,
electrophysiological examinations
conducted as Electromyoneurography
and Evoked Potentials, graft removal
site (whether from the site of the
surgery or the iliac), bone healing and
details of the aspect of the system
along the radiological monitoring
during the Rehabilitation Program.

STATISTICS
We divided the database statistical
analysis in vertical and horizontal. In
the vertical statistical analysis, we
grouped the data of the columns and
showed the analysis in Graphs. We
analyzed gender, age range, origin,
causes, level of neurological injury at
hospital admission, level of
neurological injury at discharge, deficit
in the Frankel/ASIA scale5, 7 on
hospital admission, at discharge and on
outpatient evaluation, location of
fracture or dislocation, x-rays,
classification of fractures and
dislocations according to MARGERL and
GERTZBEIN6, number of spine levels
fixed, size of the rod used, blood loss
and use or non-use of blood, average
time of surgery, details of the
procedure (decompression, iliac or site
graft, use of single, double or
multifilament wire), anteroposterior
dislocation on hospital admission and
discharge, lateral dislocation on
hospital admission and discharge,
lateral inclination on hospital admission
and discharge, correction and loss of
angulation and time between
radiological checks, days between
fracture and hospital admission, days
between hospital admission and
surgery, months between fracture and
hospital discharged after rehabilitation,
days between the surgery and
discharge, semesters between the
discharge and the last outpatient
evaluation and the FIM (Functional
Independence Measure) scale
classification on hospital admission and
discharge.
In the horizontal statistical analysis, we
used the following tests: statistical
treatment of data involving descriptive
and inferential analysis, Chi-square Test
and Fisher's Exact Test when necessary,
in order to analyze the significance of
relations among the distributions of
variables. The difference among the
groups was tested through Variance
Analysis, by using the Kruska-Wallis
Test as well. We also evaluated the
Rehabilitation Efficiency Ratio, which is
equivalent to the Functional
Independence Measure obtained at the
end of the rehabilitation program
divided by the Length of Stay in the
Program.

Rectangular Rod System with grafts
The tables below refer to a vertical
statistical analysis of results. We
observed that 95 patients (78.5
percent) were male and 26 (21.5
percent) were female (Graph 1). The
predominant age group was between
20 and 29 years, in a group of 48
patients, followed by 30 and 39 years,
with 36 patients (Graph 2), with a single
occurrence under 9 years old and two
above 60 years old. The most common
cause was traffic accident in 57
patients, followed by fall in 41 and
direct trauma in 23 (Graph 4).
The level of neurological injury at
hospital admission was in T-12 (44),
followed by T-11 (23) and L-1 in 21
cases. The other levels were affected in
a proportion of less than four. The
spinal level of fracture and/or
dislocation enables to find most of the
injuries in the dorsal lumbar transition
between T-10 and L-2. The most
frequent location of injury was T-12
with 74 patients, L-1 with 68, T-11 with
27 and L-2 with 14. As the injury often
involves two segments, the numbers
added exceed the 121 cases analyzed.
The study of the neurological level of
injury at discharge enabled an analysis
of the neurological deficit evolution of
patients (Graph 6). The neurologic
deficit evaluation in the Frankel/ASIA
scale at fracture (admission), placed 75
with total injury in A, 9 in B, 20 in C, 4 in
D and 13 in A (Graph 7). The
comparison of this evaluation with the
one at the discharge and later with the
follow up evaluation enabled the
analysis of the neurological deficit
evolution among the various severities
of injury (Graph 8).
Concerning the radiological evaluation,
all patients underwent plain x-rays.
Seventy-one examinations were
complemented with computerized
tomography and 50 with magnetic
resonance. According to the
MARGERL/GERTZBEIN (6) classification,
most patients had C-1 type fractures in
72 cases and A-3 in 63 cases (Graph
12). In the analysis of the number of

levels fixed with the surgical procedure,
we observed that 42 were fixed in 5
levels, 38 in 4, 30 in 6, 6 in 7 and 5 in 8,
and two patients had fractures in two
distant spine levels (Graph 13). The size
of the rod is proportionally related with
the number of levels fixed in most
cases: there were 55 rods of 15 cm, 50
rods of 10 cm, 15 rods of 20 cm and
only one rod of 5 cm, used in children
and made according to the size
required (Graph 14). The blood loss was
registered by volume. We noticed that
93 patients had losses of less than
1,000 ml, 85 of them did not received
postoperative transfusion, 36 patients
received transfusion in volume less or
equal to 1,000 ml of blood during the
postoperative period (Graph 15). The
average time of surgery ranged
between 1 hour and 3 hours in 87
patients with prevalence between 2
hours and 3 hours in 49 cases. In 98
patients, the decompression of spinal
canal was conducted as part of the
procedure. Patients with no
neurological deficit were excluded from
this technique (18 cases), as well as 5
other cases in which the compression
was not considered significant in the
radiological evaluation. In 62 cases, the
graft was taken from the iliac by
conventional technique and, in 59
cases, only from the site of the surgery,
including the spinous apophyses, part
of the blade, facet and vertebral body
removed at decompression and
preparation of fusion. In 71 patients,
we used double wires and, in 46
patients, the single wire was used. In
four patients, flexible multifilament
cables were used.
All cases were x-rayed during
outpatient evaluation. A new x-ray
study was conducted in 43 patients,
among the 93 patients who returned
for a new evaluation after hospital
discharge. In 93 of them, with anterior
dislocation of the vertebra, we
observed a correction from 37 percent
to 16 percent of the dislocation. In the
laterolateral dislocation, the reduction
was decreased on average from 27
percent to 19 percent. The lateral
inclination was corrected from 11 to 8
degrees. The correction and loss of
angulation were 21 degrees before the
procedure, 12 degrees immediately
after the procedure and again 21
degrees on the late evaluation that was
conducted, on average, 624 days after
the second evaluation. In 56 patients,
the time between the fracture and the
admission was, at most, 14 days, in 28
patients, 329 days and in 23 patients,
44 days.
Thirty-two procedures were conducted
until the 6th day after hospital
admission, 23 between 7 and 13 days
and 26 between 14 and 20 days of
hospital admission. We analyzed the
time between the fracture and
discharge. Among 100 patients with
apparent neurologic deficit, in 68, the
time between the surgery and the
beginning of physical therapy was less
than 14 days. For the other 32 patients,
the time was longer than the period of
14 days. For the 18 patients who could
deambulate, the average time of
discharge for deambulation was
between the 3rd and 5th postoperative
day, i.e., half of the cases. Sixty-one

patients with neurologic deficit were
rehabilitated within the 60
postoperative days and 94 patients
returned for evaluation appointment
and functional revaluation. The average
MIF Scale evaluation on admission was
29 and at discharge 79. Only patients
with brain-related injury had delays or
low final grade.
Twenty-one patients had traumatic
brain-related injury, and one of them
evolved to death due to late
intracerebral frontal hematoma.
Sixteen suffered distal fractures
(superior or inferior limbs), 7 had
abdominal injury, 3 with thoracic injury
and 2 with pelvic injury. Two patients
had fractures in two levels distant from
the thoracolumbar spine. The
complications identified before the
surgery that justified its postponement,
in addition to related traumas, such as
brain, thoracic and abdominal/pelvic
injuries were the following: urinary
tract infection in 32 patients, bedsores
in 20, venous thrombosis in 3 and
pneumonia in 1.
Occurrences identified after surgery
and related to the rod and the wires
were classified into early and late
occurrences: EARLY OCCURRENCES –
wrong level of the procedure in 1 case,
superficial infection in 2 cases that
received clinical treatment; suture
dehiscence in 2 cases, local hematoma,
despite the drain. in 2 cases, deep
infection in 2 cases, one of them
evolving to meningitis and death; LATE
OCCURRENCES - bulging on the site in 3
patients, rod exchange due to local
pain, with placement of more grafts in
2 cases, broken wire in 10 cases, 2 of
them with multifilament wire, broken
rod with no need of intervention, 1
case with previous angulation above 20
degrees, late removal of the rod for
conducting DREZ in 2 cases. Concerning
pain and spasticity, we observed that 2
patients had pain above the injury,
related to joint pain, 7 had pain at the
level of the injury and fracture site,
which was related to instability, causing
the evaluation of 2 cases in which the
rod was replaced by a larger one and
more bone graft was placed. Eight
patients complained of intense pain
below the injury, which resulted in
clinical treatment and, in 2 cases, in
conduction of DREZ. Two patients
underwent pithing, for exaggerated
spasticity with no drug control.
After the surgery, and during the period
of hospital stay, 9 patients had urinary
infection, 7 developed bedsores
(usually sacral or trochanteric
bedsores), 1 had pneumonia, 1 had
deep vein thrombosis and another one
had myositis ossificans. Several
urological complications were
identified during the outpatient follow-
up period: vesical calculi in 3 patients,
demanding surgery, hydronephrosis in
2 and stenosis of urethra in 1. Eight
patients reported penile erection
dysfunction, with indication of
prosthesis to one of them. Another
patient enrolled in the program of
fertilization and managed to be a
parent. Likewise, a paraplegic patient of
group became pregnant, with medical
monitoring, giving birth to a healthy
child.

Does the time between fracture and
surgery influence the final rehabilitation
process of patient at discharge? In Graph
33, we observe that patients who
underwent early surgery obtained a
higher number of points on the FIM
Scale at hospital discharge. This means
that they took more advantage of time
for their rehabilitation and, at discharge,
they were better trained to perform
their activities. We observed that the
average classification in the FIM Scale,
when analyzing patients who underwent
surgery up to 31 days before (45
patients), was 57.6, while in those who
underwent surgery more than 31 days
before (52 patients), was 52.8 (x²,
p=0.010), which shows the significance
of the difference. For this analysis, we
excluded patients with no neurological
deficit. We concluded that patients who
underwent early surgery obtain better
results in the rehabilitation program.
Does the time between the fracture and
the surgery influence the length of stay
of the patient in the hospital for
rehabilitation? In Graph 34, we noticed
that the later the surgery, the longer is
the length of stay of the patient in the
hospital environment. We compared
patients who underwent surgery up to
31 days before inclusively (45 patients),
and those who underwent surgery more
than 31 days before (59 patients),
concluding that the average stay was
71.2 days in the first group, and 83 days
in the second one. The difference was
significant (x², p<0.026), which
authorizes us to assert that: the shorter
the time between the fracture and the
surgery, the shorter is the length of
hospital stay. For this analysis, we
excluded the patients with no
neurological deficit.

surgery. The analysis was significant with
x², p<0.005).
Does the time between the fracture and
admission at rehabilitation hospital
influence on treatment and
rehabilitation program of the patient? In
Graph 36, we noticed that the longer the
time between the fracture and its
admission, the lower is its evaluation in
the FIM Scale at discharge. Graph 37
shows that patients admitted up to 10
days after the fracture feature shorter
hospital length of stay. This ascends
progressively between 11, 20 days, 21,
and 30, of the fracture, indicating that
the length of stay increases, reducing
gradually later. In Graph 38, the
efficiency ratio is high for patients
admitted less than 10 days after fracture
and decreases in the following groups.

By the Table 20, 30 patients were
admitted less than 10 days inclusively
and 63 more than 10 days. The
comparison of efficiency rations of these
two groups was significant (x², p<0.02),
confirming that the shorter the time
between fracture and patient admission,
the shorter is the length of stay and the
better the FIM Scale evaluation and the
efficiency ratio.

According to Graph, 39 patients were
divided into groups of 5 days, from the
date of admission to surgery, noticing
that 55 patients underwent surgery
before the 15th day of admission.
(Consider the analysis of other trauma-
related spine injuries and preoperative
complications.) In Graph 40, we may
notice that the MIF Scale gain of hospital
discharge was equal for all groups. This
indicates that the patients who waited
for surgery inside the rehabilitation
institution were included in the
rehabilitation program and could be
rehabilitated, not wasting time, even
waiting for surgery. Notice that when
comparing those patients who
underwent surgery in less than 15 days
inclusively, to those who underwent
surgery more than 15 days later, there
was no significance (x², p<0.48).
However, the length of stay of these
patients was significantly increased, as
demonstrated by Graph 41, in which the
average of stay for patients who
underwent surgery in less than 15 days
of admission inclusively was 66 days,
while those who underwent surgery
more than 15 days later was 90.7 days,
being the efficiency ratio x², p<0.000.
According to Graph 42, the efficiency
ratio decreases with the increased length
of hospital stay. It is noticed that, when
comparing 47 patients who underwent
surgery with less than 15 days of hospital
admission inclusively and 46 patients
with more than 15 days, the difference in
the efficiency ratio was highly significant
(x², p<0.005). Patients waiting for
surgical procedures of spine inside a
rehabilitation institution do not waste
time and participate in the rehabilitation
program before and after the procedure.
As the procedure is delayed, for the
treatment of other trauma-related spine
injuries, the length of hospital stay
increases, thus reducing the efficiency
ratio. (Consider the analysis of other
spine trauma related injuries and
preoperative complications.)

severe traumatic injuries of other segments? In Graph 43, a significant amount of
traumatic injuries of other segments is evident in patients admitted in the acute phase.
Influences are expected once the spinal fracture occurs in polytrauma patients. In Graph
44, patients who had and did not have other related injuries are compared.
Cranioencephalic, thoracic, abdominal, pelvic and segment injuries are included here. It
was evident that the average time between fracture and surgery in the group with
trauma in other related organs decreased from 51 days to 36 days in the group that did
not have other trauma-related injuries. The difference was significant, with p<0.05. The
trauma related to other regions decreases the indication for the surgical procedure for
thoracolumbar spine fracture.

Is the type of fracture/dislocation related to recurring angulation in late evaluation?
According to Graph 22, we notice the angulation on admission, after surgery and in the
outpatient appointment. The difference was not as significant when conducted in group,
while, in the paired analysis, the difference was significant between fractures A-2 and C-1
(p<0.05). The average of angulation loss of the fracture Type A-2 was 11 degrees and the
average of angulation loss of fracture Type C-1 was 24 degrees. This loss occurred with
the reangulation in 43 patients evaluated after discharge (third measurement) did not
reflect in the individual evaluation of their Functional Independence.
May the time between fracture and surgery be influenced by preoperative complications
such as urinary tract infection, bedsores and venous thrombosis? In Graph 45, we may
notice that, when comparing the groups with and without this type of complication, there
was no difference. The average time of indication for surgery in both groups was 41 days,
and there was no significant difference with p<0.42.

DISCUSSION
CONSERVATIVE TREATMENT OF
THORACOLUMBAR
FRACTURE/DISLOCATION
The treatment of fracture/dislocation
aims to preserve the residual function of
the spinal cord and roots and maximize
their recovery. It also aims to restore and
preserve the alignment of spinal
structures, obtain a consolidation free of
pain and instability and prevent the
occurrence of late neurologic deficit due
to angulation, also enabling to mobilize
the patient as early as possible in order
to complement the rehabilitation. Within
these principles, there is a real debate
about whether the goals preconized may
be reached with the conservative
treatment or if there is the need for
additional surgical treatment. Treatment
with postural reduction and dynamic
immobilization has been defended by
several authors with great experience8.
Laminectomy is absolutely
contraindicated in these cases, due to
the fact that it increases instability and
deformity of the spine, with late
damages9. Bed rest is recommended for
6 to 8 weeks, with use of vest for 16
weeks10. Patients with blast injury and
no deficit, or with minor deficit, may be
treated without surgery11, 12. Below
there is an example of reabsorption of
bone projection within the spinal canal
after six months of injury, where the
patient was treated without surgery
(Summary of Case 4).
The fact that those patients that
associate severe traumas of other
organs, such as skull, thorax and
abdomen, may not be treated surgically
during hospital stay, even when there is
indication for surgery, draws attention in
our casuistry. In this group, we observed
extension in the period of rehabilitation.
Those who have severe brain related
injury could not participate in the
rehabilitation program actively,
undergoing only family practice. Patients
with severe thoracic and abdomen

a major surgical procedure, were treated
conservatively within the
GUTTMANN13´s principles. It was
evidenced that the complications, which
occurred within the hospital
environment, did not extended the
length of time of the rehabilitation
program.

Summary of Case 4 - Male patient, 18
years old, running over victim when
cycling. Evolved with lumbar pain
presenting discreet dorsiflexion deficit of
the left foot with hypoesthesia in S-1. No
sphincter disorder. The computer
tomography at admission showed the
burst of L-1 with bone projection within
the canal that was treated. The
examination repeated six months after
the accident showed absorption of
anterior bone beam and progressive
remodeling of the spinal canal.
INDICATION FOR SURGERY
The indication for surgery in the spinal
cord injury has caused intense debates.
Several experimental models have been
developed over the years in an attempt
to clarify the issue. These models provide
rapid compression through bone
displacement, traction, torsion,
laceration and section, and also test the
elasticity by causing a small spinal
tension. Other models questioned the
application of compressive forces, the
kinetic energy involved, the
displacement and impulse. The results
were basically two classical models of
spinal cord injury: a dynamic model and
a static model. The dynamic model is
usually caused by rapid compression,
weight fall, application of clip or
extradural balloon. The static model uses
slow compression and constant speed,
usually caused by weight or extradural
balloon inflated slowly.
Studies such as the one by TARLOV and
KLINGER (17) used an extradural balloon
located in the lumbar region to compress
the cauda equina of dogs, causing partial
compression by 5 minutes, obtaining
recovery of neurologic deficit in 60
minutes after cessation of the
experiment. The studies by DOLAN and
TATOR (14), in addition to the one by
GUHA and TATOR (15), placed a
compression clip in the level of the first
thoracic vertebrae. Weights of 2.3g and
178g were used for 3 to 240 minutes.

of the force applied and the compression
time on the injuries considered light
(between 2.3 and 16.9g) were important
factors for the recovery of neurologic
deficit. In experiments by NYSTROM and
BERGLUND (16), in which weights of 20,
35 and 50g were applied on the thoracic
spinal cord of rats, for 1 to 19 minutes, it
was observed that the time length was
not significant with 20g, but with weights
of 35 and 50g the recovery varied with
the time length of compression. TARLOV
and KLINGER (17) proved that recovery
varied depending on the force applied
(size of the balloon) and the compression
speed (minutes or hours).
In summary, the dynamic experimental
studies proved that the force of the
injury is the main determining factor of
the neurological deficit and the length of
time may be significant for weights
considered “light”. In addition, we
should add that in static models, the
length of time of the compression was
also significant, however, the relevance
to the acute spinal cord injury is
questionable, once it is difficult to
extrapolate it to the injury in humans.
The conclusion that the neurologic
deficit is defined by the initial trauma
generated two different lines of
indication for surgery in spinal cord
injury.
Not submitting patients to surgery is a
position defended until these days by the
British School12, which initially
condemned the laminectomy in the
acute phase, whose poor outcomes were
related to greater instability. According
to Guttmann, there is no indication for
surgery, once the injury occurs at the
moment of the impact and the
decompression of medulla has no effect
on recovery. Guttmann considered, in
the occasion, that the operative
morbidity and mortality superseded the
possible benefits. Guttmann blocked,
somehow, the development of surgical
treatment with that excellent effects
obtained through the rehabilitation of
patients13. Opposing to the experience,
there is indication for early surgery,
internal fixation and immediate
rehabilitation, positioning defended by
the American and European Continent
Schools, initially influenced by
experimental studies of ALLEN BOHLLER
(62), who planned the treatment of
spinal cord injury by posterior pithing for
drainage of the hematoma. At an early
stage, Allen abandoned the research that
already tended to demonstrate the
existence of secondary injury and, thus,
provided no guidance to those who, at
the time, based on his initial studies,
indicated the surgical treatment as an
option. Over time, the two lines
developed in different directions: the
first one, with rehabilitation techniques
and postural treatment of fractures and
the second one, questioning the
existence of the secondary spinal cord
injury and the best moment to submit
the patient to surgery, whether at an
early or late stage.
The latest experimental and clinical
research suggests that the
decompression in spinal cord injury may
improve neurological recovery,
demonstrating that time is a factor of
paramount importance. DELAMARTER
(18) applied a 50 percent compression

on the spine of the dog for a period of
one week. The recovery was observed in
those uncompressed within the first
hour. Recovery is the reverse of the
compressive force applied. It is difficult
to transfer the data to the human beings.
The force applied cannot be evaluated.
We do not know if only traction is
enough to align the canal, or whether we
should conduct the total decompression
of the canal to obtain the result desired.
JOHNSSON (19) monitored 17 patients
with decompression and stabilization of
the thoracolumbar spine. The measure
of the canal area postoperatively was 71
percent of normal, with an 86 percent
improvement in monitoring. A similar
finding was reported by LEVINE (20).
However, WILLEN (21), in a study of the
natural history of burst fracture between
T-12/L-1, described that bone fragments
that occupied more than 50 percent of
the canal were not reabsorbed and,
therefore, remained reducing and
compressing the spinal cord and roots.
WATERS (23) indicates that the recovery
is more pronounced in the first 6 months
after the injury. Twelve and 24 months
after the injury, however, little
improvement occurs. These are studies
based on the American Spinal Injury
Association6 standards and according to
serial evaluation and a manual of the
muscles chosen. Myometrial surveys
seem to demonstrate continuous
recovery after 1 year of injury. In
quadriplegics, the possibility of
functional recovery decreases with the
increased distance below the level of
injury. GRAZIANI (23) found 70 percent
of recovery on the level below the injury,
only 12 percent in the second level
below the injury and 0 percent at the
third level below the injury in
quadriplegics. Muscles with partial injury
recover better than those with total
injury, in a ratio of 90 percent versus 43
percent.
BRACKEN (24) NASCIS II information
concluded that 6.7 percent of the
patients admitted with total injuries
became incomplete injuries. From those,
only ¼ could recover the motor function.
In contrast, 37 percent of the patients
admitted into Frankel B improved. Half
of them recovered some function. Fifty-
three percent of patients admitted into
Frankel C improved to a functional level.
Young patients were more successful in
the recovery than elderly patients.
There are no randomized studies in
human beings on the effects of
decompression in neurological recovery.
Publications on the subject are a mere
collection of articles relating to
contemporary or historical control
groups, or even reports on cases with no
control groups. KIWERSKI (25) examined
1,761 patients, 798 of whom did not
undergo surgery. He reported that the
groups were not similar: 20 percent of
mortality in the group of patients who
did not undergo surgery, with 8.6
percent in the group who underwent
surgery. On complete injury, the
mortality was 36 percent in the group of
patients who did not undergo surgery,
against 19 percent in the group who
underwent surgery. The hospital stay
was 27 weeks for the group of patients
who did not undergo surgery and 17 for
the group who underwent surgery. The

average length of stay in both groups
was 9 to 12 weeks. It is worth
mentioning that surgery is delayed for
those with severe related-injury or
complications resulting from traumatic
injuries of other organs. BRUNETTE (26)
showed that the traumatic cervical
dislocation reduced by traction
immediately after the injury and
appropriate radiological control may
result in significant neurological
recovery. WEINSHEL (27) evaluated a
comparative study between groups of
patients only with arthrodesis and
fixation and another one related
decompression, not noticing substantial
difference. BOSE29 analyzed patients
with center cervical spinal syndrome and
obtained sensitive improvement in the
group who underwent surgery within the
20 initial days after the injury.
KRENGEL29 evaluated decompression in
incomplete thoracic paraplegia. The
recovery obtained greater success in
patients of the historical control group,
with 68 percent of recovery in
uncompressed and fixated patients, with
a 44 percent drop in those who
underwent only the fusion. HU30
evaluated the effect of decompression in
incomplete lumbar injuries. The average
improvement in the ASIA Scale for motor
deficit was 10 points. Those patients who
had fusion with no decompression
improved 4.2 points (p>0.05), achieving
significant statistical difference.
Evidence from the literature that
confirms decompression as efficient in
the SCI (spinal cord injury) is present in
studies of Level III, IV or V (C degree).
The same recommendation is made for
the removal of fragments of the canal.
There are also two level-III studies that
did not demonstrate benefits at
decompression and removal of
fragments.
Within the protocol we used, all the
patients underwent to surgery by using
the same technique with decompression,
fixation and arthrodesis. We were able
to observe, however, that patients with
total injury kept their injury
(Frankel/ASIA = A), while those with
partial injury tended to improve. This
improvement was more evident in those
patients with injury below or including
the level of L1. We observe in Table 26-A
that 85 percent of patients in C tend to
migrate to D and E and that 55 percent
of the patients classified in B tend to
migrate to C or D. On final evaluation
(Table 26-B), 70 percent of the patients
migrated at least one level on the
Frankel/ASIA Scale.

WHEN SURGERY IS RECOMMENDED
During the surgical study, the question of
when surgery is recommended is not
resolved. The use of a new classification
of the types of fractures, as in the item
above that deals with the indication for
(early or late) surgery, does not answer
this question either. We observed the
existence of experimental studies that
favor early surgery: TARLOV (13), DOLAN
and TATOR (14), GUHA and TATOR (34)
and also studies with clinical evidence
that any recovery occurs when the injury
occurred more than 24 hours before. The
question is: what about those between 1
and 23 hours? There is also no definitive
evidence for or against early surgery.
None of the two schools could refute
injury evidence. It is present in
experimental models characterized as:
late injury in the white and grey spinal
matter, glutamate-related calcium-
mediated injury, free radicals (lipid
peroxidation) and ischemic injury. There
is also, in the literature, clinical evidence
supporting surgery. MAIMAN (33)
evaluated 20 patients with
thoracolumbar injuries that had late
decompression through lateral cavity
and noticed that 17 patients improved.
WEINSHEL (27) examined 90 patients
with cervical spinal cord injury, classified
in Frankel A and B and noticed that 47 of
them recovered the root function below
the injury and 14 have recovered the
spinal function also below the injury. We
have to consider that, in thoracolumbar
transition (most frequent location of
these fractures), the injuries are a
mixture of spinal cord and root injuries
that have different clinical behaviors.
There is also clinical evidence in the
literature that does not support the
surgery as the study by TATOR (34),
which examined 208 patients with spinal
cord injury and cauda equina, 116 (56
percent) of which underwent surgery
with mortality of 6.1 percent, compared
to 15.2 percent of patients who did not
undergo surgery, being deep vein
thrombosis the most frequent problem.
Comparing the groups, there is no
evidence of neurological recovery.
FRANKEL (5) analyzed 612 patients with
closed spinal cord injury. All were
treated with postural reduction and only
4 needed surgery for spinal instability,
which occurred lately. In this group,
there were 2 percent of neurological
worsening in patients during treatment.
MARSHALL (35) conducted a prospective
study with 283 patients with spinal cord
injury, noticing patients worsening
related to early surgery. WILLEN (8)
stated there was no difference in the
result whether the patient undergoes
early or late surgery.
Studies with clinical evidence supporting
surgery: DUH (36) studies 187 patients
with spinal cord injury, based on data
from NASCIS II, being 56 through anterior
approach, 247 through posterior
approach and 38 patients with injuries
that occurred less than 25 hours before
and improvement of 17.8 percent,
compared to the group which did not
undergo surgery, in which there was 13.8
percent improvement. In this study, 105
patients underwent surgery more than
200 hours after injury, from which 16.4
percent improved. The author concluded

that the surgery is beneficial, regardless
of the time of its conduction.
There were differences in the analysis of
the surgery effects, including misdeeds
and even mortality. For WIMOT and
HALL (37) and TATOR (34), surgery
increases the complications. Both
MARSHALL (36), who evaluated surgeries
that occurred less than 5 days after
injury, and LEVI38, who analyzed
surgeries that occurred less than 24
hours after injury, adopt the same point
of view. The study by WILBERGER (63)
analyzes a group who underwent surgery
less than 24 hours after injury, admitting
that early surgery causes fewer
complications.
The conclusions that may be obtained
from these studies are that laboratory
tests enable the assurance that
decompression may have positive
aspects, however, they raise doubts as
regards the moment to conduct such
procedure. The positioning of
GUTTMANN (12) concerning
laminectomy was indeed valid, however,
it was not consistent with the current
methods of spine fixation. Those are
techniques that may be performed
safely, in specialized centers, at any time
of the occurrence of spinal cord injury.
Several small series of cases studied
suggest the benefit of surgical
decompression, however, in contrast,
three major series, such as MARSHALL
(35), TATOR (34) and DUH (36) failed to
demonstrate the beneficial aspects.
Over time, there have been
modifications of the process. HEIDEN
(40), for example, proved that patients
with spinal injuries who underwent
surgery in the first week after injury had
46 percent of complications and for
those who underwent surgery after
more than 1 week, the ratio dropped to
27 percent. MARSHALL (35) found 15
percent of neurological deterioration in
patients who underwent surgery less
than 5 days after injury. After this period,
there was no deterioration. This allows
us to assume that the evolution of the
secondary injury, in some patients, may
be confused or aggravated by surgical
procedure, considered an additional
injury in polytrauma patients. By
analyzing the thoracolumbar region
fractures, the Sociedade para Estudo da
Escoliose (1992) showed 25 percent of
complications in patients who
underwent surgery and 3 percent in
those who did not undergo surgery.
DICKSON (41) evaluated 49 patients with
thoracolumbar fractures and found the
same recovery in a period between 8 and
35 days among those who underwent
surgery less than 24 hours after injury.
BENZEL (42) conducted anterior
decompression in patients with more
than 19 days after injury, not having
found relation between recovery and the
moment of the surgery. CLOHISY (43)
monitored 20 patients with anterior
decompression and compared half of
them with decompression in less than 48
hours to the other half with an average
of 60 days after the injury. In this series,
patients with early decompression had
better neurological recovery in
comparison to those with late
decompression.

BRUNETTE (26) has an interesting work
that studies bilateral dislocation of facets
and its reduction time with neurological
recovery. The procedure relates the early
surgical treatment with recovery. It is
also possible to construe that the
reduction of a bilateral dislocation of
facets at cervical level withdraws the
continuous traction and compression
factor of the spin cord, preventing
aggravation of the secondary injury. The
analysis detailed at NASCIS II25 shows
that from the 487 patients included in
the study, only 295 underwent surgery.
From those, 38 underwent surgical
treatment less than 25 hours after injury
and 105 more than 200 hours after
injury. In our study, we concluded that
surgery does not increase the risk of
neurological worsening. The
complications and hospital stay are
reduced, however the effect about the
neurological deficit recovery is
inconclusive.
Even among those who believe that
surgery may improve the patient with
spinal cord injury, there is some
disagreement about the ideal moment to
conduct it. WAGNER (44) is contrary to
early surgery and MARSHALL (35)
believes it may worsen the condition of
the patient. Apologists of that modality
of surgery bet on canal decompression
and spinal immobilization as factors that
enable early mobilization, thus avoiding
prolonged bed rest with neurological
improvement and early physiotherapy.
MARSHALL (35) indicates caution, once
in a multicenter study with 283 patients,
from 134 patients who underwent
surgery, 4 had the condition deteriorated
after the procedure. They underwent
surgery within the first 5 days after
injury. HEIDEN (40) observed that those
who underwent surgery in the first week
had a higher number of pulmonary
complications than those who
underwent late surgery. Improvement
was found in 1.5 to 5 percent of the
cases of those who underwent surgery,
however the time of surgery was not
analyzed. ANDERSON and BOHLMAN
(32) recommend late decompression in
cervical injuries. Forty-nine percent of
the 51 patients who underwent surgery
between 1 month and 8 years of injury
improved at least one neurological level.
KATOH and MASRI (45) evaluated 53
patients with complete cervical spinal
cord injury that were treated
conventionally and from the 40 patients
monitored for 12 months, 19 recovered.
AEBI46 evaluated 100 patients with
cervical injury and concluded that 1/3 of
them improved neurologically and 75
percent had fractures that were reduced
either manually or surgically up to 6
hours after injuries. LEVI38 examined
103 patients treated with anterior
decompression retrospectively and no
difference was observed in the
neurological recovery of the patients
who underwent surgery within 24 hours
after surgery and those who underwent
to surgery after more than 24 hours.
The literature presents a mixture of
studies of level III and V, conflicting with
each other. Based on the evaluation of
the surgical indications by DONOVAN
(47), the idea that surgery improves the
neurological outcome could not be
confirmed, however, the evidence is not

as strong to the point of invalidating his
indication. In selected patients, where
there is spine cord and roots to be
preserved, early decompression seems
to have a theoretical potential for
improvement. It is not a riskless surgery.
The patient may occasionally
deteriorate. In order to solve the
question, more detailed studies are
necessary. Thus, the moment of surgery
is still controversial, requiring a
randomized study for further
clarification, including finding out
whether the surgery is beneficial or not
in the treatment of secondary injury.
Even in the face of those facts, several
authors are in favor of early surgery,
aiming at neurological improvement with
spine stabilization and reduction of
complications related to prolonged
immobilization in bed and early
commencement of the patient
rehabilitation, with resultant cost
reduction. Those are refutable
arguments, depending on the condition
of service for the patient. In our study,
we admitted most of the patients within
the first two weeks after injury, and also
most of them undergo surgery in the
following week. The time of surgery
depends on the severity of the trauma-
related injuries. In a service aimed at
rehabilitation, there seems to be no
increase in the rehabilitation time of the
patients that wait for surgery. This is our
experience. Such conclusion is curious,
once it shows that the ones who defend
early surgery in emergency are
somewhat correct. They aim, with this,
to reduce the waiting time of the patient
to be transferred to the rehabilitation
center. On the other hand, the clinician
responsible for patients is convinced
that, with or without surgery, the
moment of the procedure does not
change the length of hospital stay and
the end result of rehabilitation. Thus, we
may be in favor of both sides in a
recurrent discussion among experts. If
the service should indicate the patient
for rehabilitation in another department,
and this transfer depends on the spine
being stabilized or not, then the waiting
time may influence the increased total
time of hospital stay. In rare and
exceptional cases, there is indication for
emergency surgery in open dura mater
injuries, progressing neurologic deficit
caused by misalignment, great
angulation or progressive angulation and
pain related with severe spinal
instability.
With reference to the conduction of the
surgery, there is disagreement in the
literature. In patients with multiple
injuries, there are authors who tend to
conduct emergency surgery, once we
know, in the early hours after injury, that
the patient has more favorable
conditions, which we call Window of
Opportunity. This window occurs before
the development of systemic
complications. Patients with spinal cord
injury, particularly those with thoracic,
abdominal or high lesion, often develop
respiratory complications (atelectasis
and pneumonia) in the hours following
spinal injury, which makes immediate
surgery inconvenient. BRACKEN (24)
demonstrated the beneficial effect of
using Methylprednisolone in the early
hours after injury and was criticized and
discredited subsequently. YOUNG and

BRACKEN (48) defend the idea that the
same therapeutic window should be
used for surgery, to improve the
prognosis of spinal cord injury recovery.
The argument would be the observation
of improvement in microcirculation and
axonal compression. Therefore, the
purpose of emergency surgery has the
following advantages: 1- it would avoid
systemic complications, stabilizing the
spine at an early stage; 2- it would
restore normal anatomophysiological
conditions at the site of injury and early
would enable the early transfer of the
patient to the place of rehabilitation.
WHICH APPROACH SHOULD BE USED
FOR SURGERY – Before the various
alternatives, posterior instrumentation
and fusion related to decompression
were selected for treatment of injuries,
especially when the surgical approach
occurs later than the 5th day after injury
(which is more frequent in this casuistry),
when only the alignment of the spine
does not enable the expansion of the
canal. There are several alternatives
regarding the surgical route to be used in
the treatment of thoracolumbar
transition fractures: anterior
decompression and instrumentation;
posterior decompression and
instrumentation and those methods
combined. The most acceptable option is
controversial. Mc AFEE and BOHLMAN
(49) concluded that, with the anterior
decompression of thoracolumbar spine,
there was improvement of one level in
the Frankel Scale in 88 percent of
patients with incomplete neurological
injury. KANEDA (50) obtained, through
the same anterior route, an
improvement of one level in 70 percent
of patients. DICKSON and HARRINGTON
(51) obtained improvement of level in
the Frankel Scale in 75 percent of
patients who underwent surgery by
posterior approach. HARDAKER52
conducted bilateral pedicle trans
decompression and instrumented with
Harrington rods, obtaining improvement
of one level in 85 percent of cases.
EDWARDS (53) instrumented through
posterior approach and all of his patients
had neurological improvement. ESSES54
conducted a prospective study and
compared two groups, with no
difference between the anterior and
posterior approaches. GERTZBEIN (55)
did not notice statistical difference and
88 percent of patients who underwent
surgery through anterior approach
improved one level in the Frankel Scale,
while 83 percent of those who
underwent surgery through posterior
approach also improved one level. In our
study, we observed that 50 percent of
patients with partial deficit (Frankel/ASIA
B/C/D) migrated at least one level in the
clinical evaluation at discharge. Among
46 patients, 23 improved. In the
evaluation appointment after surgery,
other 6 patients of this group showed
upward migration in one level, increasing
the improvement ratio to 70 percent.
SHAFFREY and DANISA56 conducted a
prospective study with 130 patients,
from whom 49 underwent surgery, 27
through posterior approach, 16 through
anterior approach, and 6 through the
two routes. Among the items compared,
the blood loss, the hospital stay, the cost
of the procedure and the time of surgery
were higher in the anterior approach.

However, the correction of kyphosis and
the neurological improvement were not
significantly different among the groups.
In the posterolateral approach through
facetectomy, the fenestration enables
the removal of anterior bony projections
and disc material often herniated to the
inner part of the spinal canal. The
fixation system consisting of a rectangle
and sublaminar wires enables a rigid
construction, which provides fixation and
consolidation of arthrodesis. DOVE (64)
should include abundant amount of bone
graft on the entire region fixed and not
on the level fractured. This leads to a
lower stress on the system, reducing the
indication for late surgery to withdraw
the system due to fatigue or rupture. In
patients with persistent compression of
the spinal canal and with no
improvement of the neurological deficit
as expected, the anterolateral approach
in a second moment may be chosen. If
fusion through posterior approach has
already occurred, there is no need for
instrumentation, and only for
decompression with bone graft.
WHICH APPROACH SHOULD BE USED
FOR SURGERY
Before the various alternatives,

for treatment of injuries, especially when
the surgical approach occurs later than
the 5th day after injury (which is more
frequent in this casuistry), when only the
alignment of the spine does not enable
the expansion of the canal. There are
several alternatives regarding the
surgical route to be used in the
treatment of thoracolumbar transition
fractures: anterior decompression and
instrumentation; posterior
decompression and instrumentation and
those methods combined. The most
acceptable option is controversial. Mc
AFEE and BOHLMAN (49) concluded that,
with the anterior decompression of
thoracolumbar spine, there was
improvement of one level in the Frankel
Scale in 88 percent of patients with
incomplete neurological injury.
KANEDA50 obtained, through the same
anterior route, an improvement of one
level in 70 percent of patients. DICKSON
and HARRINGTON (51) obtained
improvement of level in the Frankel Scale
in 75 percent of patients who underwent
surgery by posterior approach.
HARDAKER52 conducted bilateral pedicle
trans decompression and instrumented
with Harrington rods, obtaining
improvement of one level in 85 percent
of cases. EDWARDS (53) instrumented
through posterior approach and all of his
patients had neurological improvement.
ESSES54 conducted a prospective study
and compared two groups, with no
difference between the anterior and
posterior approaches. GERTZBEIN (55)
did not notice statistical difference and
88 percent of patients who underwent
surgery through anterior approach
improved one level in the Frankel Scale,
while 83 percent of those who
underwent surgery through posterior
approach also improved one level. In our
study, we observed that 50 percent of
patients with partial deficit (Frankel/ASIA
B/C/D) migrated at least one level in the
clinical evaluation at discharge. Among

46 patients, 23 improved. In the e
valuation appointment after surgery,
other 6 patients of this group showed
upward migration in one posterior
approach, 16 through anterior approach,
and 6 through the two routes. Among
the items compared, the blood loss, the
hospital stay, the cost of the procedure
and the time of surgery were higher in
the anterior approach. However, the
correction of kyphosis and the
neurological improvement were not
significantly different among the groups.
In the posterolateral approach through
facetectomy, the fenestration enables
the removal of anterior bony projections
and disc material often herniated to the
inner part of the spinal canal. The
fixation system consisting of a rectangle
and sublaminar wires enables a rigid
construction, which provides fixation and
consolidation of arthrodesis. DOVE (64)
should include abundant amount of bone
graft on the entire region fixed and not
on the level fractured. This leads to a
lower stress on the system, reducing the
indication for late surgery to withdraw
the system due to fatigue or rupture. In
patients with persistent compression of
the spinal canal and with no
improvement of the neurological deficit
as expected, the anterolateral approach
in a second moment may be chosen. If
fusion through posterior approach has
already occurred, there is no need for
instrumentation, and only for
decompression with bone graft.
COMPLICATIONS
In the posterior and posterolateral
approach, there may be dural injury in
the preparation or placement of the
instruments and, whenever possible, it
should be repaired. The placement of
instrumental may compress the spinal
cord and/or roots, accentuating deficit
and pain. Adequate training for the use
of instrumentation and the preoperative
use of the evoked, and motor and
sensory potentials may be useful for the
prevention of this type of complication.
There may be bad reduction,
hypertraction, hyperextension or
rotation. We indicate the conduction of
radiological control during surgery. The
procedure is conducted with the
prophylactic use of antibiotics. The
patient preparation is important as well
as special attention to skin conditions in
the preoperative stage. Infection may
mean the withdrawal of the instruments,
which should be postponed to the
maximum, i.e. until spinal fusion. Major
bleeding is possible during the
procedure. It is a complication that must
and may be avoided by careful
dissection, ligation or protection of
vessels. The use of drains in the surgical
area and a routine, one drain in the
surgery site and another one in the side
of graft donation, when applicable.
In the anterior portion approach, there
may be a number of problems: injuries of
abdominal and thoracic organs, due to
proximity to the spine, bleeding by
laceration of the inferior vena cava,
aorta or smaller arteries, neurological
injury and liquoric fistula, in addition to
infectious complications, with formation
of pseudarthrosis. Special attention

should be given to dissection plans and
hemostasis. The great vessels should be
dissected, deviated or connected before
being sectioned.
Occasionally, there may be other distant
complications, as in the pneumothorax
due to trauma-related fracture of ribs,
pulmonary embolism and urinary
infection. Late complications may
happen with the disruption of the system
used for fixation, due to bending
strength. The postoperative pain may be
due to the deafferentation, when a few
patients may require the conduction of
DREZ (coagulation of posterior root
entry). This means later approach, with
removal of the entire system and
conduction of a broader laminectomy.
Such a procedure, if possible, should be
conducted later, after the occurrence of
the anterior fusion, once this may lead to
instability.
CONCLUSIONS
Patients with severe neurologic deficit
related to unstable thoracolumbar spine
injuries are potential candidates for
surgical procedures.
The moment to conduct the surgical
procedure should respect all clinical
conditions of the patient. Individuals
with injury related with skull, thorax and
abdomen should have their surgery
scheduled for the moment the general
condition is clinically balanced. The time
waiting for surgery does not influence
the outcome with regard to neurological
recovery and rehabilitation as
demonstrated in this study. It only
extends the length of hospital stay and
reduces the hospital efficiency ratio
which is a function based on the
rehabilitation time.
The method to be used should enable
the concomitant decompression of
neural elements with spinal fixation and
arthrodesis. The decompression of
neural elements should be considered
whenever there is deficit and/or
misalignment of the spinal canal. The
fact that lumbar injuries show recovery
potential five times higher than when
compared with thoracic and cervical
injuries justify this approach, since those
are mixed spinal root injuries.
The surgical stabilization of the spine
occurs immediately. The method relieves
pain and stabilizes the deformity,
enabling the complementation of the
rehabilitation program. Late evaluation
enabled to detect that patients admitted
with great dislocations and treated by
using this method evolve with
reangulation at the level of the injury
and it does not influence the final
outcome of treatment and rehabilitation.
Preoperative clinical complications did
not change the rehabilitation program of
the patient, but influenced in deciding
when to indicate the surgical procedure.
The postoperative complications are
similar to those of other methods, and
are treatable.
Safety, simplicity and economy are
factors that contribute to surgeons
continue using this method to treat
thoracolumbar spine fractures and/or
dislocations.

HIDROCEFALIA EN PACIENTES PEDIATRICOS CON TUMOR DE FOSA POSTERIOR: PROPUESTA DE ESCALA PREDICTIVA

RESUMEN
El objetivo del estudio fue evaluar variables asociadas al desarrollo de hidrocefalia en pacientes atendidos en el Instituto Nacional de Salud del Niño y elaborar una escala predictiva. Se revisó 69 Historias Clínicas de pacientes menores de 18 años admitidos entre enero 2009 y diciembre 2012 con diagnóstico de tumor de fosa posterior y sometidos a posterior resección quirúrgica. Se investigó las variables edad, sexo, localización y tamaño tumoral, edema transependimario, metástasis y diagnóstico patológico. Se excluyeron pacientes con registro inadecuado, ausencia de verificación radiológica o muerte antes de 6 meses de seguimiento. Las variables edad < 3 años, localización tumoral a nivel de línea media/IV ventrículo/tronco cerebral, edema transependimario y presencia de metástasis conformaron la escala y se les asignó una puntuación de 1 a 3. Ésta fue aplicada a los pacientes considerándose bajo riesgo de desarrollo de hidrocefalia puntajes de 1 a 4 y alto riesgo puntajes ≥ 5. Los pacientes fueron evaluados en consulta ambulatoria y sometidos a control radiológico cuando existía sospecha clínica de hidrocefalia. Solo se consideró el desarrollo de hidrocefalia dentro de los primeros 6 meses de seguimiento. El sexo predominante fue el masculino (59.42%) y la edad media fue 6.49 años ±DS 3.8. El 69.56% desarrollaron hidrocefalia, 44.93% como forma de presentación del tumor y 24.63% post resección. El 92.3% de < 3 años desarrolló hidrocefalia lo cual fue significativo (p: 0.04284). El 83.93 % con tumores de localización en línea media, IV ventrículo y tronco desarrollaron hidrocefalia y constituyó la variable más significativa (p: 0.000000370). De igual forma el tamaño tumoral (p: 0.0009676) y el edema transependimario (p: 0.0009676). El tipo tumoral no tuvo relación con el desarrollo de hidrocefalia. El 97.37% con puntajes ≥ 5 desarrollaron hidrocefalia frente a 35.48% con puntajes de 1-4 (RR: 2.744 y p: 0.0000001). El 78.95% de aquellos con hidrocéfalo post resección tuvo puntaje ≥ 5. La estratificación de riesgo mediante la aplicación de la escala permitiría la detección precoz de pacientes con alto riesgo de desarrollo de hidrocefalia, favoreciendo una intervención temprana, así como evitaría la morbilidad asociada a procedimientos en pacientes con riesgo bajo.

ABSTRACT
The aim of the study was to evaluate variables associated with the development of hydrocephalus in patients treated at the National Institute of Child Health and develop a predictive scale. A retrospective chart review of 69 patients under 18 admitted between January 2009 and December 2012 with diagnosis of posterior fossa tumor and subsequently underwent resection. The variables age, sex, tumor location and size, transependymal edema, metastasis and pathological diagnosis was investigated. Patients with inadequate registration, absence of radiological verification or death within 6 months follow up were excluded. Variables age < 3 years, tumor location at midline/ IV ventricle/brain stem, transependymal edema and metastases make up the scale and assigns a numerical value between 1 and 3. This was applied to patients and considered lower risk of developing hydrocephalus scores of 1 to 4 and higher scores ≥ 5. Patients were evaluated on an outpatient basis and under radiological control when hydrocephalus was suspected. Only the development of hydrocephalus within the first 6 months of follow up was considered. The majority of patients were male (59.42%) and the mean age was 6.49 years ± SD 3.8. Hydrocephalus was present in 69.56%, 44.93% as clinical presentation of tumor and 24.63% post resection. The 92.3% of <3 years developed hydrocephalus which was significant (p= 0.04284). The 83.93 % with tumors located at midline/ IV ventricle/brain stem developed hydrocephalus and was the most significant variable (p= 0.000000370). Likewise, tumor size (p: 0.0009676) and transependymal edema (p = 0.0009676). The tumor type was not related to development of hydrocephalus. The 97.37 % with scores ≥ 5 developed hydrocephalus versus 35.48 % with scores of 1-4 (RR: 2.744 and p = 0.0000001). The 78.95% of those with hydrocephalus post resection had score ≥ 5. Risk stratification by applying the scale would allow early detection of patients at high risk of developing hydrocephalus, promoting early intervention and avoid the morbidity associated with procedures in patients with low risk.

INTRODUCCION
Debido a la proximidad al IV ventrículo y a las vías de circulación del Líquido Cefalorraquídeo (LCR) todo paciente pediátrico con tumor de fosa posterior está predispuesto al desarrollo de hidrocefalia. La necesidad real de un sistema de derivación de LCR permanente antes o después de la cirugía tumoral en dichos pacientes es desconocida. No existe aún un consenso, incluso, en el tipo de procedimiento quirúrgico a emplear en los casos de hidrocefalia con expresión clínica.
La hidrocefalia obstructiva asociada a tumores de fosa posterior ha sido reportada en 70-80% de casos y es frecuentemente la forma de
presentación clínica28,29. Los reportes varían en la incidencia de hidrocefalia post resección siendo de 18 a 40%1. De ello deriva que la generalización del uso de una derivación permanente de LCR u otro tipo de procedimiento expone a alrededor de 70%6 de pacientes a procedimientos que no están exentos de riesgos. En ese sentido la familiarización con la derivación ventriculoperitoneal (DVP), su colocación relativamente sencilla, el limitado uso de técnicas endoscópicas (por falta de insumos, curva de aprendizaje en proceso, limitados recursos imagenológicos, entre otros) y la información aun poco clara sobre los beneficios de técnicas endoscópicas en este tipo de pacientes han hecho de la derivación permanente

de LCR un recurso frecuentemente elegido.
Por otro lado, existen reportes que demuestran que la realización de una tercer ventriculostomia endoscópica (TVE) reduce el riesgo de hidrocefalia de 26.8 a 6%2 en pacientes pediátricos con tumores de fosa posterior. Su realización constituiría una vía fisiológica para la corrección de la hidrocefalia asociada a tumores de fosa posterior. A ello se suma la reducción en el riesgo de metástasis abdominales a través del catéter de derivación de LCR reportada hasta en 10-20%8 de casos y la eliminación de la morbilidad asociada a la implantación de un shunt permanente estimada en alrededor de 40%9.
La persistente controversia de tratar o no tratar profilácticamente la potencial hidrocefalia asociada a tumores de fosa posterior derivó en la preocupación de la elaboración de una escala predictora de desarrollo de hidrocefalia post resección confiable y sobre todo capaz de ser aplicada a todas nuestras realidades, objetivo del presente estudio. Esta tuvo como punto de partida la Canadian Preoperative Prediction Rule for Hydrocephalus3, modificada por variables de fácil aplicabilidad en cualquier centro hospitalario independiente de su complejidad.
La escala incluyó edad < 3 años, localización tumoral respecto a la línea media, tamaño tumoral ≥ 4 cm, presencia de edema transependimario y presencia de metástasis al momento del diagnóstico, consignados por la literatura como factores predictores de desarrollo de hidrocefalia, a las cuales se les evaluó estadísticamente. Esta fue aplicada a toda la población en estudio para evaluar su capacidad predictora.
El uso extendido de la escala en la población pediátrica portadora de un tumor de fosa posterior permitirá la estratificación de riesgo de desarrollo de
hidrocefalia y la realización de una derivación “profiláctica” permanente de LCR concomitante a la resección tumoral, a través de una TVE de eficacia y de morbilidad menor demostrada2 o de una DVP en aquellos centros sin acceso a las técnicas endoscópicas. De igual forma ayudaría al médico y familia en la toma de decisiones.
MATERIALES Y METODOS
Estudio retrospectivo que incluyo la revisión de 69 Historias Clínicas de pacientes menores de 18 años admitidos consecutivamente en el Instituto Nacional de Salud del Niño entre enero 2009 y diciembre 2012 con el diagnóstico nuevo de tumor de fosa posterior (con demostración radiológica) y que fueron sometidos a resección quirúrgica. Se investigaron las variables de edad, sexo, la localización tumoral respecto a la línea media, el tamaño tumoral, la presencia de edema transependimario, la presencia de metástasis al momento del diagnóstico y el diagnóstico patológico. La variable edad y tamaño tumoral fueron dicotomizadas para su mejor manejo.
Se excluyeron pacientes con registro inadecuado de información o verificación radiológica en el seguimiento que desarrollaron hidrocefalia posterior al alta hospitalaria o la muerte antes del periodo de estipulado para descartar hidrocéfalo (6 meses).
Aquellos pacientes dados de alta sin el desarrollo de hidrocefalia durante su hospitalización fueron seguidos en su evolución para evaluar el desarrollo de hidrocefalia de necesidad de un sistema de derivación permanente de LCR. En aquellos pacientes la utilización de algún tipo de shunt o manejo endoscópico de la hidrocefalia previa a cirugía fue criterio de exclusión para su consideración como hidrocefalia post resección.

El tamaño tumoral fue medido en su máximo diámetro en un corte tomográfico axial de las imágenes preoperatorias. La localización tumoral fue medida como la distancia entre el centro geométrico del tumor y la línea media3. Si dicho centro se encontraba medial a la mitad del radio de la fosa posterior el tumor era considerado de localización medial.
El tipo tumoral (presunción radiológica) no fue incluido como criterio debido a la poca sensibilidad que acarrea los hallazgos tomográficos de tumores de fosa posterior frente a los hallazgos mediante resonancia magnética y sus diversas secuencias5. Ello teniendo en cuenta, además, que solo una minoría tuvo y tiene acceso a las imágenes por resonancia magnética en nuestro instituto. También fue excluida la evaluación de la hidrocefalia bajo los criterios de leve, moderada o severa por
ser de carácter muy subjetivo. Es su reemplazo, el ya demostrado edema transependimario4, fue utilizado como variable predictora de desarrollo de hidrocefalia pre y post resección.
Las variables con mayor potencia estadística de acuerdo a la literatura actual (edad < 3 años, localización tumoral a nivel de línea media/IV ventrículo/tronco cerebral, edema transependimario y presencia de metástasis) fueron seleccionadas para la elaboración de la escala predictiva. A dichas variables se les asignó una puntuación de 1 a 3. La suma de la valoración total de la escala tuvo un mínimo de 1 y un máximo de 10 puntos. Dicha escala fue aplicada a todos los pacientes y se consideró de bajo riesgo de desarrollo de hidrocefalia aquellos pacientes con puntajes de 1 a 4 y alto riesgo aquellos con puntajes ≥ 5. (Tabla 1)

Los pacientes fueron regularmente evaluados clínicamente en consulta ambulatoria y sometidos a control radiológico para descartar la presencia de hidrocefalia cuando existía sospecha clínica, de acuerdo al protocolo del servicio. Solo se tuvo en consideración el desarrollo de hidrocefalia dentro de los primeros 6 meses post resección tumoral, considerándose como positivo
(hidrocefalia post resección) la necesidad de manejo quirúrgico de la hidrocefalia, independiente del procedimiento empleado (DVP o TVE).
El análisis univariante fue realizado mediante X2 entre la variable independiente y el desarrollo de hidrocefalia. En aquellas variables en las cuales la probabilidad de un resultado esperado era menor a 5 se utilizó la

prueba exacta de Fisher. Los resultados de la puntación de la escala de cada paciente fueron convertidos en variables dicotómicas (bajo y alto riesgo) para su manejo.
RESULTADOS
La evaluación mediante los criterios de inclusión y exclusión permitió seleccionar y evaluar 69 historias clínicas. El sexo predominante fue el masculino 41 (59.42%) y la edad media fue de 6.49 años ±DS 3.8. El 18.84% (13) fueron menores de 3 años.
Del total el 69.56% (48) desarrollaron hidrocefalia tributaria de manejo quirúrgico. De ellos 31 (44.93%) como forma de presentación de la patología al momento de ingreso y 17 (24.63%) en el periodo post operatorio. El 30.43% (21)
no desarrollo hidrocefalia durante el ingreso hospitalario ni durante el periodo de seguimiento ambulatorio de 6 meses.
El 92.3% (12) de los pacientes menores de 3 años desarrolló hidrocefalia lo cual tuvo significancia estadística (p: 0.04284). El 83.93 % (47) de los pacientes con tumores de localización en línea media, IV ventrículo y tronco desarrollaron hidrocefalia igualmente (p: 0.000000370) y constituyó la variable con mayor significancia estadística. De igual forma el tamaño tumoral (p: 0.0009676) y el edema transependimario (p: 0.0009676) fueron estadísticamente significativas. La presencia de metástasis cerebrales al momento del diagnóstico no resultó con significancia estadística (p: 0.4808). (Tabla 2)

posible el diagnóstico patológico debido a la decisión de manejo paliativo por la localización tumoral (tronco cerebral) y en un caso debido al no tratamiento quirúrgico por retiro voluntario del paciente de la institución. El 73.68% (28) de pacientes con meduloblastoma desarrollaron hidrocefalia, la gran mayoría (18) en la etapa pre-resección, sin embargo, no fue estadísticamente significativo (p: 0.6778). No fue significativo, igualmente en los casos de pacientes con ependimomas.
El 97.37% (37) de los pacientes con puntajes ≥ 5 en la escala predictora desarrollaron hidrocefalia antes o después de la resección tumoral frente al 35.48% (11) de los pacientes con puntajes de 1-4 (RR: 2.744 y p: 0.0000001). Del total de pacientes que desarrollo hidrocefalia durante el seguimiento ambulatorio el 78.95% (15) tuvieron puntaje ≥ 5. En aquellos que desarrollaron hidrocefalia no hubo diferencia estadística entre la probabilidad de desarrollarla antes o después de la resección tumoral.(Tabla 02)

DISCUSION
A pesar de la elevada incidencia de hidrocefalia obstructiva asociada a tumores de fosa posterior que oscila entre 70 a 80%, la necesidad real de una derivación permanente de LCR se reporta entre 11.5 a 39% de casos de acuerdo a la literatura actual2,10,22,28. Esta relativa baja incidencia no justificaría el uso rutinario y generalizado de una derivación permanente del LCR por los riesgos potenciales propios del procedimiento, entre los que se encuentran infecciones, disfunción, obstrucción, migración o extrusión del catéter, convulsiones y perforación visceral. De igual forma, y no menos importante, la necesidad obligatoria de servicios médicos a largo plazo en todo paciente portador de un shunt.
En el manejo urgente de un hidrocéfalo agudo asociado a un tumor de fosa posterior, se ha descrito también como complicación de la derivación del LCR la herniación del vermis superior a través
del borde libre del tentorio, así como la hemorragia intratumoral por disminución rápida de la presión intracraneal, pudiendo producir ambas un deterioro neurológico agudo.
Además de la morbilidad descrita previamente, la colocación generalizada de una derivación permanente de LCR previa a la resección tumoral propuesta inicialmente por algunos autores11,12,13 es una alternativa observada hoy en día debido al riesgo de metástasis abdominales a través del catéter de derivación de LCR8, y la mayor sobrevida de pacientes pediátricos con tumores de fosa posterior en la última década14,15,16,17,26 que ha colocado en discusión la calidad de vida que brindamos a estos pacientes. A ello se suma el hecho demostrado que la colocación de un shunt “profiláctico” en un paciente que no lo necesita acarrea 2.5 veces más riesgos3.

La calidad de vida en pacientes pediátricos con tumores de fosa posterior constituye hoy en día uno de los objetivos del tratamiento27. Esta descrito el compromiso de múltiples campos como el cognitivo, físico, social y psicológico, especialmente en pacientes sometidos a radioterapia como parte del tratamiento complementario. A ello se suma que la implantación de una derivación permanente de LCR se ha asociado a una disminución en la calidad de vida27 y constituye un predictor negativo de sobrevida debido a la morbilidad asociada a su implantación.
A pesar de los beneficios atribuidos a la colocación de una derivación ventricular externa transitoria anterior a la resección tumoral como la reducción de la presión intracraneal, el relajamiento de la fosa posterior previo a la apertura dural, el monitoreo y manejo post operatorio de la presión intracraneal, el drenaje de LCR sanguinolento y disminuir la probabilidad de fístula de LCR, la recomendación de su colocación18,19,20 se ha relacionado con una mayor incidencia de desarrollo de hidrocefalia post resección en pacientes pediátricos7.
El 69.56% de nuestros pacientes necesitaron de una derivación permanente de LCR pre resección tumoral, situación que contrasta con los reportes actuales 28,29 y nos obligaría a la revisión de nuestros criterios para manejo el hidrocefalia asociada a tumores de fosa posterior a la luz de la literatura actual. El retraso en la cirugía debido a la desorganización de los procesos o la logística, la dependencia de la “libertad de camas” en la Unidad de Cuidados Intensivos, la DVP como un recursos frecuentemente utilizado y con el que la gran mayoría de cirujanos cuenta con amplia experiencia serian los
principales factores condicionantes de esta cifra.
El porcentaje de pacientes que desarrollaron hidrocefalia post resección (24.63%) en nuestro estudio no distan de los hallazgos de Culley et al.21 quien además demostró que a pesar de una resección tumoral amplia y la visualización intraoperatoria del restablecimiento del flujo de LCR, hasta 31% de pacientes desarrollan hidrocefalia post operatoria. Este hallazgo tendría relación con que la cirugía de fosa posterior es frecuentemente complicada por meningitis aséptica la cual se acompaña del desarrollo de aracnoiditis y formación de adhesiones o membranas a nivel de la salida del IV ventrículo. Ello podría deberse igualmente al desarrollo de metástasis post resección que obstruyan el normal flujo de LCR, entre otras causas.
Las consideraciones previas obligan al desarrollo de estudios que permitan la determinación de aquellos factores de riesgo capaces de predecir el desarrollo de hidrocefalia en pacientes pediátricos con tumores de fosa posterior. Diversos estudios se han abocado en ese objetivo e incluso han desarrollado escalas predictivas3,4 como la desarrollada en el presente trabajo. Sin embargo, al igual que la escala nuestra, su aplicación en realidades diferentes o en establecimientos de diferente nivel de complejidad podría no constituir la mejor herramienta.
En nuestro estudio a mayor puntaje en la escala mayor la probabilidad de desarrollo de hidrocefalia pre o post resección tumoral. No hubo diferencia estadística entre la probabilidad de desarrollar hidrocefalia antes o después

de la resección tumoral lo cual demostraría la capacidad de las variables que conforman la escala como predictores confiables y de utilidad en la práctica clínica diaria.
Los escasos recursos con los que cuenta las Instituciones Públicas Hospitalarias de nuestro país, y algunos países de Latinoamérica, hace muchas veces imposible la toma de exámenes como la resonancia magnética que permita una mejor valoración pre operatoria de las lesiones tumorales y un adecuado plan quirúrgico, teniendo muchos de nuestros pacientes que ser intervenidos solo con los hallazgos tomográficos (62% de nuestros pacientes). De allí la importancia del desarrollo de una escala predictiva capaz de aplicarse a todas nuestras realidades independientemente de la complejidad de la Institución Hospitalaria.
La estratificación de riesgo de pacientes mediante la aplicación de la escala predictora permitiría la detección precoz de aquellos pacientes con alto riesgo de desarrollo de hidrocefalia favoreciendo una intervención temprana a través de una derivación “profiláctica” de máximo beneficio y de morbilidad menor frente a los riesgos de una hidrocefalia hipertensiva aguda y una derivación tardía del LCR. Dicha derivación, a través de una TVE2 o de una DVP11,12,13, ha sido recomendada previamente en otros estudios. La TVE fue propuesta por Chumas24 en el manejo de la hidrocefalia secundaria a tumores de fosa posterior y su eficacia definida en estudios posteriores2. Esta disminuiría el riesgo de desarrollo de hidrocefalia post resección tributaria de una DVP25.
La estratificación de riesgo también sería de utilidad en el seguimiento de
pacientes que no desarrollaron hidrocefalia pre o post resección, sustentando solamente un seguimiento clínico sin exposición radiológica en aquellos pacientes con bajo riesgo de desarrollo de hidrocefalia, disminuyendo con ello la exposición a radiación ionizante en población pediátrica. Tumores cerebrales inducidos por radioterapia han sido reportados con bajas y altas dosis de radiación, si bien la relación con la realización de tomografías cerebrales es aún poco clara30,31.
Diversas publicaciones han demostrado que la edad es uno de los principales factores de riesgo asociada al desarrollo de hidrocefalia en pacientes pediátricos con tumores de fosa posterior2 ,22,23, especialmente en menores de 3 años7,21, hallazgo con significancia estadística determinado igualmente en el presente trabajo.
Los pacientes con localización tumoral cercana a línea media, IV ventrículo o tronco cerebral muestran mayor riesgo de desarrollo de hidrocefalia debido a la cercanía y mayor probabilidad de afectar las vías regulares de circulación del LCR, alterando su dinámica. Dicho compromiso se debería al efecto de masa de la lesión tumoral inicialmente y luego podría deberse al trauma quirúrgico derivado de la exéresis tumoral. Es decir a mayor tamaño tumoral mayor trauma quirúrgico. Dicha observación fue corroborada en nuestro estudio y fue la variable con mayor significancia estadística, siendo consistente con lo encontrado en otros estudios7.

variables derivadas del proceso de atención hospitalaria, entre otras, pueden influenciar el desarrollo de hidrocefalia post resección en relación a un mayor grado de resección tumoral7 y la consecuente descompresión y liberación de las vías de normal circulación del LCR.
El edema transependimario es de fácil reconocimiento en una tomografía convencional y un factor predictor de riesgo de hidrocefalia demostrado por Foreman y su grupo de estudio4. Por ser un hallazgo tomográfico de fácil reconocimiento, incluso por ojos poco experimentados, es una variable más confiable y menos sujeta a las variaciones inter-observador frente a la valoración subjetiva del tamaño ventricular4, o índices como el de Evans o el índice del cuerno frontal u occipital utilizados en otros reportes7.
La presencia de metástasis al momento del diagnóstico del tumor de fosa posterior también ha sido demostrada como un fuerte predictor de desarrollo de hidrocefalia3,4,23. Sin embargo, debido al pequeño número de casos encontrado con metástasis en nuestro estudio (2 casos) no es posible inferir conclusiones significativas.
Finalmente, la escala propuesta sería de utilidad clínica debido a que está conformada por variables fácilmente medibles, medibles de manera consistente, pueden ser obtenidas prontamente al ingreso y permiten una estratificación. Sin embargo, la naturaleza fisiológica entre las relaciones establecidas puede ser sólo teórica y debido al tamaño muestral carecer de la suficiente evidencia3, de allí la recomendación de su validación mediante un estudio prospectivo futuro.
CONCLUSIONES
Los factores predictores del desarrollo de hidrocefalia en pacientes pediátricos con tumores de fosa posterior son edad < 3 años, localización tumoral en línea media, IV ventrículo o tronco cerebral, tamaño tumoral ≥ 4 cm y presencia de edema transependimario.
La escala elaborada tiene una elevada capacidad predictora y fácil aplicabilidad. Su uso extendido en la población pediátrica portadora de un tumor de fosa posterior permitirá la estratificación de riesgo de desarrollo de hidrocefalia y la realización de una derivación “profiláctica” permanente de LCR concomitante a la resección tumoral, a través de una TVE o de una DVP en aquellos centros sin acceso a las técnicas endoscópicas. De igual forma, se evitaría la morbilidad asociada al procedimiento en aquellos pacientes con riesgo bajo. Esta estratificación favorecería, igualmente, el consentimiento informado y ayudaría al médico y familia en la toma de decisiones.
Se recomienda la validación de la escala propuesta con una población independiente en un diseño prospectivo y que incluya un mayor número de participantes.

RESUMEN
El absceso cerebral consiste en una área central supurativa dentro del parénquima cerebral,
dentro de una envoltura ampliamente vascularizada. Los patógenos ampliamente aislados en
la patogénesis de esta entidad son los Streptococos spp. No obstante el avance en la terapia
antimicrobiana, las técnicas neuroquirúrgicas y la TAC, que permite su diagnóstico y ubicación
oportuna, el absceso cerebral aún se considera un problema de salud pública, con una
importante incidencia, morbilidad y mortalidad en países en desarrollo (menor al 20%). Para el
manejo de este tipo de infección del SNC, se requerirá de un abordaje multidisciplinario que
involucre terapia médico quirúrgica. El objetivo de esta revisión es hacer un abordaje amplio
sobre la patobiología del absceso cerebral relacionada con la labor concerniente al
neurocirujano.

ABSTRACT
Brain abscess is formed as a central suppurativa area within the brain parenchyma, within an envelope extensively vascularized. Microorganisms largely isolated in the pathogenesis of this condition are Streptococcus spp. Despite the progress in antimicrobial therapy, neurosurgical techniques and TAC, which enabling timely diagnosis and location, brain abscess is still considered a public health problem and has an important incidence, morbidity and mortality in developing countries (less than 20%). To handle this type of CNS infection, will require a multidisciplinary approach involving surgical medical therapy. The aim of this review is to make a comprehensive approach on the pathobiology of brain abscess related to the work concerning the neurosurgeon.

INTRODUCCIÓN
El absceso cerebral (AC), se constituye en una infección del sistema nervioso central, cuyo curso ha cambiado debido al avance técnico en terapia antimicrobiana. Se define como una infección focal dentro del parénquima cerebral, que tiene su inicio como un área localizada de cerebritis y que posteriormente continua como una colección de pus dentro de una cápsula bien vascularizada (1) En el siglo XIX eran una entidad catastrófica, con muy mal pronóstico, cuyo tratamiento quirúrgico estaba reservado para un grupo selecto de individuos(2). Su patogénesis, clínica, tratamiento y pronóstico fue magníficamente descrita por en 1893 por el médico Sir William MacEwen, en su monografía sobre infecciones del sistema nervioso central (SNC). MacEwen obtuvo resultados extraordinariamente positivos en los abscesos que intervino quirúrgicamente, con una mortalidad de sólo 5%, lo cual confirmó para el mundo que el absceso cerebral no complicado, operado a tiempo, la recuperación debe ser la regla. Pese a ello, las tasas de mortalidad registradas en los años 70 llegaban hasta el 50%. Desde entonces, gracias fundamentalmente al diagnóstico precoz con la TAC cerebral, estas tasas de mortalidad han descendido hasta situarse en los 90 entre el 5-10% (3) Sabemos que hoy en día, esto ha cambiado y el advenimiento de las nuevas terapias antimicrobianas, avances en imagenología y manejo quirúrgico permiten hoy que esta, sea una entidad que permite un manejo favorable y accesible a la mayoría de la población. Es una entidad con amplia etiología y patogenia, siendo su diagnóstico difícil de establecer,
especialmente al comienzo del curso de la enfermedad. No obstante el avance de la tecnología imagenológica y tratamiento antibiótico, los abscesos cerebrales se constituyen aún en una patología muy importante, con una morbilidad y mortalidad elevada. (3) Debe diferenciarse de las infecciones parameníngeas, absceso epidural y empiema subdural, que pueden originar oclusión de los senos venosos y venas cerebrales contiguas, causando trombosis venosa cerebral(4) No debe confundirse con las infecciones parameníngeas, absceso epidural y empiema subdural, que pueden originar oclusión de los senos venosos y venas cerebrales contiguas, causando trombosis venosa cerebral(2) EPIDEMIOLOGÍA
El uso de terapia inmunosupresora y con corticosteroides y antibióticos de amplio espectro tiene una importante relevancia en el aumento de la incidencia de la adquisición de abscesos cerebrales de origen micótico. Anterior al advenimiento del virus de inmunodeficiencia humana (VIH), se estimaban entre 1.500 – 2.500 casos anuales tratados en los EE.UU. con una incidencia estimada del 0,3 – 1,3 casos por cada 100.000 personas. Y mientras algunos autores creen que su incidencia ha tenido un leve incremento en los últimos años, otros afirman que ha permanecido relativamente estable durante la era antibiótica (2) Los abscesos cerebrales como complicación de la craneotomía ocurren con una frecuencia del 1 al 8% según informes del Departamento de Neurocirugía de la Universidad de Minnesota, EE.UU., y del Departamento de Neurocirugía del Hospital Charing Cross de Londres(5) En la mayor parte de las series, la relación hombre mujer es de 2:1

aproximadamente, con una edad media de presentación de 30 – 40 años, ocurriendo el 25% en niños menores de 15 años, mayormente en el grupo etario comprendido entre los 4 y 7 años de edad. La edad de distribución varía dependiendo de la condición predisponente que conlleve a la formación del absceso cerebral. Cuando el absceso se relaciona con un foco infeccioso paranasal, la mayoría de los pacientes se encuentran en el rango de edad de 10 – 30 años de edad; mientras que si el foco es otogénico, los pacientes generalmente son menores de 20 años y mayores de 40. Usualmente los casos que se presentan en niños , se originan a partir de focos otogénicos o enfermedad cardíaca congénita cianosante, siendo extremadamente rara su aparición en menores de 2 años. En alrededor del 15 – 30% de los abscesos cerebrales se desconoce la causa desencadenante . En la tabla 1 se muestran las condiciones, que según la literatura mundial, predisponen más frecuentemente al desarrollo de abscesos cerebrales. La incidencia de secuelas neurológicas en pacientes que sobreviven a esta entidad se halla en el rango de 20 – 70%, siendo la mortalidad de 8 – 25% . Las diversas series muestran que el pronóstico está primeramente determinado por la rapidez de progresión de la enfermedad antes de la hospitalización, el estado inmunológico del huésped y el estado mental durante la admisión (Glasgow < 12, mal pronóstico) .
Etiología
La etiología de los abscesos cerebrales es principalmente polimicrobiana. Los estreptococos y bacterias anaeróbicas son los patógenos más frecuentemente
aislados. (6) Según algunos estudios, el tejido cerebral sería más susceptible al desarrollo de infección en comparación a otros tejidos, lo que se estudió en un modelo de rata de absceso cerebral experimental. Las inyecciones de 10 unidades formadoras de colonias (CFU) de S. aureus o 106 UFC de E. coli no lograron causar la infección en la piel, mientras que el tejido cerebral era susceptible a 102 UFC de ninguno de los organismos, con formación de abscesos resultantes(2). La patogenia de los abscesos cerebrales está relacionada con el foco primario de infección, características inmunológicas y edad del paciente y la predisposición del mismo a contraer infecciones del sistema nervioso. (3) Entre los microorganismo más comúnmente identificados como productores de abscesos cerebrales se distinguen los patógenos anaerobios, originados muy frecuentemente de la flora oral bacteriana normal(7), y asimismo de infecciones otorrino-laringeas, también pueden diseminarse a través de lesiones intrabdominales o pélvicas, proviniendo del colon y tracto genital femenino. Los estos patógenos anaerobios, los más comunes son estreptococos anaerobios; Bacteroides spp (erobios y anaerobios), incluyendo Bacteroides frágilis, Prevotella melaninogénica; Propionilbacterium; Fusobacterium; Eubacterium; Veillonella y Actinomyces.
También se han identificado
ampliamente los patógenos aerobios, y
Gram positivos , entre los cuales se
incluye el Estreptococo viridans;
Estreptococo milleri, el cual es
característicamente identificado, ya que
presenta enzimas proteolíticas que
inducen a la necrosis tisular y formación

de abscesos (8). También se distinguen,
Estreptococo microaerofílicos;
Estáfilococo aureus y raramente
Estreptococo pneumoniae. El
Estafilococo aureus como agente causal
es más a menudo aislado posterior a un
trauma o procedimientos
neuroquirúrgicos. Los microorganismos
aerobios como los Rhodococos equi,
raramente pueden asociarse a abscesos
cerebrales, principalmente en pacientes
inmunosuprimidos.
Los microorganismos aerobios
Gramnegativos son aislados
generalmente en lesiones cerebrales
secundarias a neurocirugía o trauma
craneoencefálico, o a partir de una
infección ótica. Dentro de este grupo
los patógenos más frecuentes son
Klebsiella pneumoniae; Pseudomonas
spp; Escherichia coli y Proteus spp. Son
menos frecuentes Haemophilus
aphorophilus; Actinobacillus
actinomycetemcomitans; Salmonella y
Enterobacter spp. Los abscesos
cerebrales por Klebsiella pneumoniae,
en presencia o no de meningitis,
pueden signos de metástasis infecciosas
originadas de un absceso hepático
primario.
En el caso de los pacientes inmunocomprometidos, el rango de microorganismos es mayor en comparación a los pacientes inmuocompetentes. La Listeria monocitogenes es un microorganismo característico de este grupo, causal de abscesos únicos o múltiples, principalmente en aquéllos que reciben tratamiento prolongado con corticosteroides. La mortalidad es tres veces mayor en comparación con la
mortalidad por abscesos cerebrales de otra etiología(9) Otro patógeno aerobio es la Nocardia asteroides, la cual llega al parénquima cerebral a través de diseminación hematógena, habitualmente a partir de un foco infeccioso respiratorio. Entre los agentes de naturaleza micótica se encuentran el Aspergillus; Cryptococo neoformans y el Coccidioides immitis, que luego de colonizar el aparato respiratorio invaden el sistema nervioso central. El Aspergillus como causante de absceso cerebral es poco común, su diagnóstico y tratamiento continúa siendo un desafío. La mayoría de los hongos son organismos poco virulentos, a menudo viven como comensales. Sin embargo, representan un reto imponente en un huésped inmunocomprometido. Bhaskar y col. presentaron el caso fatal de la aspergilosis intra-craneal en un paciente inmunocomprometido con esteroides sistémicos (10). Otros hongos causales son la Cándida albicans; Mucormicosis; Cladosporium trichoides y Curvularia spp. Estos agentes patógenos son de muy difícil aislamiento y en su mayoría causan abscesos múltiples. Dentro de la población con distintos tipos de inmunodepresión, se encuentran los pacientes con serología positiva para HIV; quienes con cierta frecuencia presentan lesiones que pueden simular absceso en el parénquima cerebral con múltiples diagnósticos diferenciales, ya sea de origen infeccioso o neoplásico, como el linfoma primario de sistema nervioso central(11), entre estos pacientes también se ha identificado como causante de absceso cerebral la Legionella micdadei (12).

Toxoplasma gondii es la causa más común de absceso cerebral por protozoos(2).También se distinguen parásitos como la Taenia solium, generadora de cisticercosis, ocasionada por la forma larvaria de la Taenia solium, es una de las principales causas de las lesiones cerebrales en países en desarrollo (9). Otros parásitos menos comunes como causantes de abscesos cerebrales son el Trypanosoma cruzi, Entamoeba histolítica, Shistosoma japonicum y el Paragonimus species. (2) Los abscesos cerebrales por Propionibacterium acnes son poco frecuentes. Es importante para el médico clínico la rápida identificación de este patógeno para la elección de una terapéutica antibiótica adecuada. P. acnes es un patógeno infrecuente como causal de abscesos cerebrales, sin embargo no se debe desestimar en muestras neuroquirúrgicas.(5) Los abscesos cerebrales son complicaciones poco frecuentes de meningitis bacteriana o sepsis en recién nacidos y lactantes. En los recién nacidos se distinguen varios microorganismos. Escherichia coli rara vez se ve como un patógeno en absceso cerebral a esta edad. Pero Erdemir y copartícipes reportan un caso de absceso cerebral en bebés gemelos causadas por sepsis por E. coli. Este caso se explicó por la presencia de una mutación genética de receptores heterocigóticos Toll-like 4 (TLR4). La cual interviene en la respuesta natural a productos de bacterianas e inicia la respuesta inmune específica contra estos agentes patógenos.(13) Nocardia nova es una especie infrecuente como causa de infección en humanos. Describimos un paciente inmunocompetente con abscesos
cerebrales múltiples en quien se aisló esta bacteria durante un drenaje quirúrgico de la colección. Su recuperación fue satisfactoria tras el drenaje y una terapia prolongada con cotrimoxazol.(14) El Streptococcus intermedius es un pertenece al grupo de Streptococcus anginosus, s ele conoce también como el grupo de Streptococcus milleri. Aunque este es un agente comensal de la boca y las vías respiratorias superiores, se ha reconocido como un patógeno importante en la formación de abscesos. Aunque rara vez se ha involucrado en la formación de un absceso cerebral en los niños, se han presentado algunos casos. S. intermedius debe ser considerado como un potencial patógeno involucrado en el desarrollo de absceso cerebral en los niños.(15) Historia Natural de la enfermedad Varios modelos animales han sido utilizados para examinar las consecuencias fisiopatológicas y el curso temporal de abscesos cerebrales después del inicio de la infección. Se utilizó un modelo canino para definir las etapas patológicas de la formación de un absceso cerebral después de la inoculación de α-hemolíticos. Llevando a cabo una evaluación histológica detallada, se definieron cuatro etapas de la evolución absceso cerebral: cerebritis temprana (día 1 al 3), cerebritis finales (días 4 a 9), la formación de cápsula temprana (día 10 a 13), y la formación de cápsula tardía (día 14 y posterior a la de inoculación inicial). Aunque estas etapas son arbitrarias, resultan útiles en la clasificación y en la comparación de los organismos con respecto a su virulencia

en la producción de un absceso cerebral. La etapa de cerebritis temprana se caracteriza por un infiltrado inflamatorio agudo con bacterias visibles sobre el edema de la tinción de Gram y marcada que rodea la lesión. El centro de la lesión se vuelve necrótico durante la etapa de cerebritis tardía.(2) Fisiopatología Los microorganismos (bacterias, hongos y/o parásitos) pueden alcanzar el cerebro por diferentes mecanismos (Ver tabla 2). El más común es la propagación desde un foco infeccioso contiguo (40 – 50% de los casos) , más frecuentemente el oído medio, las celdas mastoideas o los senos paranasales .Los abscesos que ocurren posterior a otitis media se localizan usualmente en el lóbulo temporal o el cerebelo . Cuando se desarrollan secundarios a sinusitis paranasal, el sitio predominante es el lóbulo frontal, al igual que cuando se presentan infecciones dentales. En casos de sinusitis esfenoidal, frecuentemente implica el lóbulo temporal o la silla turca Un segundo mecanismo para la formación de abscesos cerebrales, es la diseminación hematógena al cerebro desde focos distantes de infección (20 – 30% de los casos). Cuando esto se presenta, usualmente se constituyen abscesos múltiples con una mayor mortalidad que los anteriores. Las fuentes más comunes de infección inicial en los adultos son las enfermedades pulmonares piogénicas, especialmente abscesos pulmonares, bronquiectasias, empiema y fibrosis quística . También pueden ocurrir a partir de infecciones de la piel,
osteomielitis, infecciones pélvicas, colecistitis e intraabdominales. (2) El trauma es un tercer mecanismo patogénico en el desarrollo del absceso cerebral (10 – 25% de los casos) . Este ocurre secundariamente a una fractura craneal abierta con ruptura de la duramadre, o como resultado de neurocirugía o lesión con cuerpo extraño (2). También se pueden presentar abscesos cerebrales en el contexto una enfermedad cardiaca congénita (CHD) en niños y adolescentes, dicha asociación es rara en adultos pero se ha registrado casos.(16) También se han observado abscesos cerebrales en el contexto de enfermedades de base, vena cava persistente superior izquierda en un hombre de 58 años de edad.(17) En un estudio observacional de una cohorte de pacientes adultos con absceso cerebral (AC) ingresados en un hospital de 944 camas, se estudiaron las tipologías epidemiológicas, clínicas y microbiológicas de los pacientes con AC, la localización y evolución de los AC, en un período de 30 años dividido en 2 mitades: 1976-1989, período 1 (P1) y 1990-2005, período 2 (P2). Se estudiaron a 108 pacientes con AC (66 en P1 y 42 en P2), con una edad media de 45 años (extremos: 12-86). El 42,4% de los pacientes tenían más de 40 años de edad en P1 y el 71,4% en P2 (p < 0,05). Gómez y copartícipes hallaron que la incidencia anual fue de 4-5 y 2-3 casos por 106 habitantes por año en ambos períodos. Ellos lograron identificar el foco primario de infección en el 86% de los casos, con diferencias estadísticamente significativas entre ambos períodos en el origen primario (otitis media: el 18,2 frente al 2,4%; infección dental: el 3 frente al 16,7%; traumatismo craneoencefálico: el 16,7 frente al 0%; posterior a neurocirugía: el 15,1 frente al 21,4%, en P1 y P2,

respectivamente). No obstante algunas diferencias epidemiológicas y clínicas entre ambos períodos y la menor incidencia en el P2 Gómez y colaboradores concluyeron que el comportamiento de AC no ha variado en los últimos 30 años. (18) La embolización endovascular se ha convertido en una opción de tratamiento importante para aneurismas cerebrales junto con el recorte quirúrgico. Pero pocas literaturas mencionaron complicaciones infecciosas después de embolización de aneurismas. No obstante, Chen y col. describen un caso de un absceso cerebral como complicación de embolización endovascular. (19) El absceso cerebral se distingue también como una complicación posterior a un procedimiento neuroquirúrgico. Representa un problema grave y requiere atención inmediata.(20) También se puede observar como una complicación común de una afección cardiaca congénita (CHD) por sus siglas en inglés, en niños y adolescentes. Esta asociación es poco común en adultos. (16) CONTEXTO CLÍNICO El curso clínico del absceso cerebral varía desde una forma indolente hasta una fulminante. La mayoría de las manifestaciones clínicas no se deben a signos sistémicos de infección, pero sí al tamaño y localización de una lesión ocupante de espacio dentro del cerebro, y la virulencia del microorganismo causante. La cefalea es el síntoma más comúnmente presentado en la mayoría de los pacientes (70 – 75% de los pacientes) (ver tabla 3). Puede variar según el grado de intensidad y también en su patrón de localización, de
generalizada a hemicraneal, lo que generalmente se asocia a un retraso en el diagnóstico. Un empeoramiento repentino de esta, acompañado de meningismo de novo, podría significar la ruptura del absceso hacia el espacio ventricular; complicación frecuentemente asociada con una alta tasa de mortalidad (85% en algunas series)(2). La localización del absceso define la presentación clínica. En la afección del lóbulo frontal predominan además de la cefalea, letargo, déficit de atención, deterioro del estado de conciencia, hemiparesia con signos motores unilaterales, y trastornos del habla. La presentación clínica del absceso cerebeloso incluye ataxia, nistagmus, vómitos y dismetría. Los del lóbulo temporal pueden causar cefalea ipsilateral y afasia si la lesión se halla en el hemisferio dominante; también un defecto del campo visual (por ejemplo, cuadrantopsia homónima superior) podría ser el único signo clínico de un absceso en esta localización(2) . Cuando se hallan en el tallo encefálico usualmente se manifiestan con debilidad facial, fiebre, cefalea, hemiparesia, disfagia y vómitos. Los hallazgos clásicos de un síndrome bien definido del tallo encefálico están usualmente ausentes en los pacientes con abscesos en esta localización, ya que estos últimos tienden a extenderse longitudinalmente a lo largo de las fibras nerviosas, mientras que los primeros lo hacen transversalmente. Ciertos agentes patógenos pueden conducir al desarrollo de manifestaciones clínicas específicas. Así por ejemplo, especies de Aspergillus, pueden generar cuadros clínicos con signos de un enfermedad cerebrovscular (ya sea isquémica, hemorrágica o ambas) (2). Por otro lado las lesiones por candidas incuyen

microabcesos, macroabcesos, granulomas no clasificantes y nódulos gliales difusos. Factores de Riesgo Para abscesos cerebrales por hongos: prematuridad, comorbilidad, neutropenia, enfermedad granulomatosa crónica, diabetes Mellitus, alteraciones de temperatura, cateter venoso central. (2) La incidencia de la infección humana causada por T. gondii depende de los hábitos alimenticios (especialmente la cantidad de carne consumida y si comido rara, en bruto, o bien hecho), el número de gatos callejeros que viven en la proximidad de las personas, las condiciones climáticas (temperaturas moderadas y humedad favorecen la supervivencia de alta ooquistes en el suelo), y el nivel general de saneamiento e higiene. Diagnóstico Se elabora por medio de la clínica sugestiva de AC, marcadores séricos aumentados e imagenología. La tomografía axial computarizada (TAC) con medio de contraste y la resonancia magnética (RM) son primordiales en el diagnóstico por neuroimagen, el cual a su vez es fundamental. La TAC proporciona localización con una sensibilidad del 95 al 99%(21) número, tamaño y estadío de la lesión, la cual se observará como una zona de lesión hipodensa, con un halo periférico (igual en la RM), asimismo permitirá hidrocéfalo, hemorragia intracranial, edema e infecciones asociadas (22,23). La TAC de doble contraste es útil en estadios tempranos del AC en los cuales la inmadurez de la cápsula, propia de
esta etapa entorpece su visualización. (23) Hallazgos en la RM Es útil en el diagnóstico diferencial del AC con otras entidades. En el T2–ponderado se observa una cápsula hipointensa, edema perilesional hiperintenso, y el centro de la lesión puede ser isointenso o hiperintenso, En imágenes ponderadas por difusión, el AC se deja ver hiperintenso y con coeficientes de difusión bajo. En el T1– ponderado un AC es de centro hipointenso, e hiperintenso en la T2/FLAIR. La pared es hiperintensa en el T1–ponderado e hipointensa en T2 (21) RM ponderada en T1 con gadolino en plano coronal que evidencia expansivo cortico- subcortical con anillo gruesco irregular de realce con centro necrótico. TRATAMIENTO MÉDICO Las infecciones de cabeza y cuello se constituyen en una emergencia y puede tener resultados repentinamente fatales sin no se hace un reconocimiento y tratamiento oportuno (25, 25, 27).

Requiere un abordaje multidisciplinario que implique la participación de intensivistas, neurocirujanos, radiólogos y especialistas en enfermedades infecciosas. Los agentes antimicrobianos utilizados para tratar el absceso cerebral bacteriana debe ser capaz de penetrar en la cavidad del absceso y deben tener actividad in vitro frente a los patógenos aislados. Los pocos estudios que se han ocupado de la penetración de los antibióticos en el cerebro absceso líquido han incluido un número limitado de pacientes (2). Mientras que se inicia la terapia antibiótica empírica, particularmente en pacientes con sepsis o herniación, deben hacerse todos los esfuerzos para obtener rápidamente el diagnóstico microbiológico y/o histológico. Es importante mencionar que hasta la actualidad, no existe una guía internacionalmente aceptada para el manejo del absceso cerebral, y muchos asuntos de interés permanecen sin resolverse, incluyendo la selección de los pacientes para tratamiento conservador, el régimen antibiótico más apropiado, la ruta y tiempo de administración, el rol de los nuevos antibióticos, etc . Por el riesgo de herniación cerebral (en caso de grandes lesiones), podría necesitarse tratamiento del edema cerebral incluso antes de iniciar neurocirugía (dexametasona intravenosa, 16 – 24 mg /día dividida en cuatro dosis). Los corticoides frecuentemente disminuyen el edema dentro de 8 horas pero retardan la formación de la cápsula alrededor del absceso, pueden suprimir la respuesta inmune a la infección y además disminuir la penetración del antibiótico; por lo tanto, deben usarse por cortos períodos, usualmente solo hasta la descompresión quirúrgica . (28,29,39)
El tratamiento óptimo para los abscesos cerebrales hongos por lo general requiere un enfoque médico y quirúrgico combinado, la cirugía consiste en la escisión o el drenaje del absceso. Terapia de absceso cerebral por hongos en pacientes inmunocomprometidos conlleva una alta tasa de mortalidad, sin embargo, a pesar de la cirugía y la terapia antifúngica. Sin embargo, la detección temprana de esta infección puede conducir a un resultado satisfactorio, sobre todo si el recuento de leucocitos vuelven a la normalidad o si las dosis de inmunosupresores puede reducirse(2) Los esteroides sistémicos se utilizan comúnmente en el tratamiento de diversas condiciones inflamatorias. A pesar de su perfil relativamente seguro, un peligro de tal tratamiento es la supresión inmune.(10) El pilar de la terapia médica para la candidiasis absceso cerebral es una preparación de anfotericina B más 5-flucitosina. La eficacia de fluconazol en el tratamiento de Candida absceso cerebral no ha sido evaluada, aunque un informe de casos en un bebé prematuro con Candida albicans absceso cerebral mostró una disminución en el tamaño absceso después de la adición de fluconazol a la anfotericina B más 5-flucitosina. Para absceso cerebral por Streptococcus Milleri el uso de metronidazol y cefotaxima, está de conformidad con las recomendaciones recientes de la Sociedad Británica de Quimioterapia Antimicrobiana y tiene buenos resultados. remoción de cuerpos extraños y astillas
de hueso, en el caso de los ubicados en
cerebelo o en tallos hay un gran riesgo
de herniación por lo que se suele
realizar de manera urgente
craneotomía de fosa posterior.
La primera modalidad de tratamiento
quirúrgico es la aspiración o drenaje del
absceso, por ser simple y por poderse
realizar bajo anestesia local. Permite
obtener muestras y aliviar la HIC,
desafortunadamente hasta en un 70%
de los casos se necesita reaspiración y
siempre existe el riesgo potencial de
ruptura intraventricular. Una adecuada
descompresión y el descarte de
hemorragia intracerebral producto de
una cápsula friable pueden asegurarse
con la RM intraoperatoria.
CONCLUSIONES
El absceso cerebral es una entidad que
pone en alto riesgo la vida y requiere de
acciones médicas rápidas y precisas que
permitan un pronóstico favorable. El AC
continúa siendo un problema de salud
pública en países en desarrollo y un
reto para el neurocirujano. Es prudente
que el personal médico sea consciente
del riesgo de este tipo de infecciones
oportunistas en pacientes
inmunocomprometidos, que puede
hacer más difícil su tratamiento.
El abordaje oportuno, los correctos
recursos imagenológicos, la terapia
antibiótica, y la neurocirugía para el
tratamiento de esta patología, permiten
al equipo multidisciplinario obtener
buenos resultados. El paso primero
para el tratamiento de esta entidad,
será evitarla, reduciendo los factores de
riesgos y poniendo especial atención en
poblaciones vulnerables a contraer este
tipo de infecciones.

RESUMEN:
Objetivos: Determinar la situación sociofamiliar, valoración funcional y enfermedades prevalentes de los adultos mayores que acuden a los establecimientos de salud del primer nivel de atención de la Dirección de Salud del Callao. Materiales y métodos. Estudio transversal, realizado con 301 adultos mayores, a quienes se evaluó con un cuestionario basado en la ficha única de Valoración Clínica del Adulto Mayor establecida por el Ministerio de Salud del Perú. Resultados. La mayoría (69,8%) de la población eran mujeres (44,5%) entre 60 y 69 años; 31,9% fueron categorizados como adulto mayor frágil. Un 79,4% de adultos mayores es independiente, 46,2% se encuentra en situación de riesgo social y 28,9% tiene problema social. La mayoría (66,1%) carece de un seguro de salud. Las enfermedades prevalentes fueron hipertensión arterial (42,5%), artrosis (33,6%) y el antecedente de tuberculosis estuvo presente en el 6,6%. Las mujeres tuvieron una peor situación sociofamiliar (p<0,05) y la valoración funcional y clínica se deterioró con la edad (p<0,05). Conclusión. La mayoría de los adultos mayores evaluados se encuentran en riesgo sociofamiliar, por lo que es necesario establecer estrategias de intervención como programas de seguridad social que contribuyan al mejoramiento de la atención integral de salud de las personas adultas mayores.Materiales y Métodos: Se realizó una revisión biográfica de Pio del Rio Hortega focalizada en su vida en Argentina. Se tomaron fotografías de preparados neuropatológicos que él realizó en ese país y se seleccionó un número representativo de ellos.

Las mujeres tuvieron una peor situación sociofamiliar (p<0,05) y la valoración funcional y clínica se deterioró con la edad (p<0,05).
Conclusiones: La mayoría de los adultos mayores evaluados se encuentran en riesgo sociofamiliar, por lo que es necesario establecer estrategias de intervención como programas de seguridad social que contribuyan al mejoramiento de la atención integral de salud de las personas adultas mayores.
ABSTRACT
Objectives: To determine social and family situation performing a functional assessment and looking for prevalent diseases in older adults attending primary care health centers in Callao, Peru.
Materials and methods. A cross-sectional study performed in 301 older adults who were administered a questionnaire based on the form for the clinical assessment of older adults established by the Peruvian Ministry of Health.
Results. Most of the persons were women, 44.5% , between 60 and 69 years old; 31.9% were classified as frail elderly persons. 79.4% of this elderly population is independent, 46.2% are at social risk, and 28.9% actually have social problems. The most persons (66.1%) do not have any health insurance. The prevalent diseases were hypertension (42.5%), arthrosis (33.6%), and a past history of tuberculosis was found in 6.6%. Women had a worse social and family situation (p<0.05), and their functional and clinical assessments findings worsened with age (p<0.05).
Conclusion. The most older adults are at a social and family risk, so it is necessary to establish interventional strategies, such as social security programs aiming to contribute with the improvement of integrated health care in this population.
INTRODUCCIÓN
A nivel mundial el envejecimiento de la población es un hecho de especial importancia por sus implicancias económicas y sociales. En nuestro país la población de adultos mayores viene aumentando significativamente y está ligado a la baja de la mortalidad y al aumento de la longevidad.
En el Perú como en el resto de países de América Latina el proceso de envejecimiento se caracteriza porque
se produce en un contexto de pobreza e inequidades, con una baja cobertura de la seguridad social y una probable tendencia hacia la disminución del tamaño de las fuentes de apoyo producto de los cambios en la estructura y composición familiar.
El envejecimiento es un proceso deletéreo, progresivo, intrínseco y universal que con el tiempo ocurre en todo ser vivo a consecuencia de la

interacción genética del individuo y su medio ambiente. Los cambios que se producen en la vejez se relacionan con las enfermedades crónicas, la malnutrición, marginación social, perdida prematura de su capacidad funcional. (1)
La organización Mundial de la Salud (2), considera personas mayores a aquellas que tienen 60 o más años y ha definido la salud de las personas en relación al mantenimiento de la capacidad funcional, clasificándolas en tres grandes grupos:
– Persona adulta mayor auto Valente: Aquella capaz de realizar actividades básicas de la vida diaria como: comer, vestirse, asearse y de continencia. Se considera que el 65% de los adultos mayores que viven en la comunidad tiene esta condición.
– Persona Adulta mayor frágil: aquella que tiene algún tipo de disminución del estado de reserva fisiológica asociada con un aumento de susceptibilidad a discapacitarse. Se considera que el 30% de adultos mayores que viven en la comunidad tiene esta condición.
- Persona adulto mayor dependiente o postrada: aquella persona que tiene una perdida sustancial del estado de reserva fisiológico, asociada a una restricción o ausencia física o funcional que limita o impide el desempeño de las actividades de la vida diaria, se considera que entre el 3% a un 5 % de las personas mayores que viven en la comunidad tiene esta condición
Según proyecciones de las Naciones Unidas, para mediados de este siglo América Latina y el Caribe tendrán 112 habitantes de 60 años de edad o mayores por cada 100 de 15 años o
menores, y en América del Norte esta razón será de 148 a 100.(3)
De acuerdo a las proyecciones demográficas de la Comisión Económica para América Latina y el Caribe (CEPAL), en las próximas décadas aumentarán aun más las tasas de crecimiento de la población mayor de 60 años de edad, de manera que en 2025 este grupo de edad será de alrededor de 100 millones de personas y en 2050, de cerca de 200 millones. Por lo que los adultos mayores de 60 años que representaban el 8,1% de la población total de América Latina en el año 2000 serán el 14,1% en 2025 y representando cerca de la cuarta parte de la población total (23,4%) en 2050. (4).
En el Perú se estima que el 53.4% de la población de 60 y más años de edad esta conformado por mujeres. La esperanza de vida en el varón actualmente es de 66 años y en las mujeres es de 71 años; para el quinquenio 2005-2010 será de 69 y 74 años respectivamente. (5)
Actualmente los adultos Mayores que acuden a los establecimientos de salud del Callao pueden ser clasificados en 4 categorías a través de la aplicación de la Ficha Única de la Valoración Clínica del Adulto Mayor (VACAM) (6), que debe ser aplicada una vez en el año, de acuerdo a la Norma Técnica de Salud para la Atención Integral de Salud de las Personas Adultas Mayores Nº 043-MINSA/DGSP-V01-2006 vigente.
Las cuatro categorías de clasificación de la Persona Adulta Mayor son los siguientes:

El objetivo principal del presente estudio fue determinar las Enfermedades Prevalentes, Valoración Funcional y Situación Socio-Familiar de los Adultos Mayores que acudieron a los Establecimientos de Salud de la DISA I Callao. Así mismo ha permitido categorizar al adulto mayor de acuerdo a Norma técnica de Atención Integral.
Materiales y Métodos
TIPO DE ESTUDIO
Se realizó un estudio observacional,
descriptivo, transversal y prospectivo
en los 48 establecimientos del I nivel de
atención de la DISA I Callao entre junio
y agosto del 2006.
POBLACIÓN Y MUESTRA
Se incluyeron a todos los adultos
mayores que acudieron a consulta en
los establecimientos de salud de la DISA
I Callao durante el mes de Julio del
2006 en el horario de atención
establecidos, se excluyeron a los que
requirieron los servicios de emergencia.
Se diseñó un muestra probabilística,
estratificada de afijación proporcional
al número de adultos mayores, se tomó
como población de referencia al
número de adultos mayores atendidos
durante el mes de Julio del 2005 en los
centros y puestos de salud de la DISA I
Callao. Se consideró un intervalo de
confianza del 95% y una tasa de no
respuesta de 7%, necesitando una
muestra de 301 adultos mayores.
Se seleccionó al Adulto Mayor que
acudía a la consulta externa en forma
sistemática cada dos atenciones hasta
completar la carga asignada al
establecimiento de salud.
RECOLECCIÓN DE DATOS
Se empleó un cuestionario semi
estructurado basado en la Ficha de
Atención Integral del Adulto Mayor
contenida en la Norma Técnica del
Ministerio de Salud del Perú
(referencia), personal de salud
capacitado recopiló los datos mediante
entrevista directa con el adulto mayor
o su acompañante.
El cuestionario contenía información
sobre datos generales del AM y de los
miembros de su familia, antecedentes
de salud, valoración funcional y
valoración sociofamiliar del AM.
DEFINICIONES
Se clasificó a los pacientes de la
siguiente forma (6)
PAM activa saludable. Es aquella sin
antecedentes de riesgos, sin signos ni
síntomas atribuibles a patologías
agudas, sub. Agudas o crónicas y con
examen físico normal.
PAM enferma. Es aquella que presenta
alguna afección aguda, sub. Aguda o
crónica, en diferente grado de

gravedad, habitualmente no
invalidante, y que no cumple los
criterios de Persona Adulta Mayor
Frágil o Paciente Geriátrico Complejo.
PAM frágil. Es aquella que cumple dos
o más de las siguientes condiciones: 80
años a más, dependencia parcial (Índice
de Katz), deterioro cognitivo leve o
moderado (Test de Pfeiffer),
manifestaciones depresivas (Escala
abreviada de Yesavage), riesgo social
(Escala de Valoración Socio – familiar),
una caída en el último mes ó más de
una caída en el año, pluripatología (tres
ó más enfermedades crónicas),
enfermedad crónica que condiciona
incapacidad funcional parcial (Ej. déficit
visual, auditivo, osteoartritis, secuela
de ACV, enfermedad de Parkinson,
EPOC, insuficiencia cardiaca reciente, y
otros), ppolifarmacia (Toma más de
tres fármacos por patologías crónicas y
por más de 6 semanas),
hhospitalización en los últimos 12
meses, Índice de Masa Corporal (IMC)
menor de 23 ó mayor de 28.
Paciente geriátrico complejo. Es aquel
que cumple con tres o más de las
siguientes condiciones: 80 años a más,
pluripatología (tres o más
enfermedades crónicas), el proceso o
enfermedad principal tiene carácter
incapacitante (Katz: dependencia total),
deterioro cognitivo severo, existe
problema social en relación con su
estado de salud (Escala de Valoración
Socio – familiar), paciente terminal (ej.:
neoplasia avanzada, insuficiencia
cardiaca terminal, otra de pronóstico
vital menor de 6 meses).
ANALISIS DE DATOS:
Se estimaron las frecuencias y
porcentajes respectivos y para medir la
asociación de algunas variables se
utilizó la Prueba de Chi Cuadrado al
95% de confianza.
RESULTADOS
CARACTERÍSTICAS DE LA POBLACIÓN
Se entrevistaron a 301 adultos
mayores, 210 (69,7%) fueron mujeres,
la edad promedio fue de 71 (+-x) años,
220 (63,1%) tenían educación primaria
o eran analfabetos, 135 (43,8%)
refirieron estar casados o conviviendo
con su pareja. Un tercio (33,9%) de los
adultos mayores que buscan atención
en los establecimientos de salud de la
DISA Callao cuentan con un seguro de
salud.

VALORACIÓN SOCIO-FAMILIAR
Sólo 24,9% tiene una valoración socio-familiar de buena o aceptable. El sexo esta
asociado al nivel de valoración socio-familiar (p<0,05), los varones tienen una mejor
situación que las mujeres (29,7 vs. 22,9%), 50,9% de las mujeres se encuentran riesgo
social. No se encontró relación con el grupo de edad.

La mayoría (71,4%) vive con familiares y sin conflicto familiar; sin embargo 3,1% se
encuentran en situación de abandono familiar, 38,5% no reciben pensión ni otros
ingresos y 46,2% no cuenta con seguro social (Tabla 2).

VALORACIÓN CLÍNICA Y ENFERMEDADES PREVALENTES DEL ADULTO MAYOR
Solo 13,6% de los adultos mayores son saludables, 23,9% son enfermos, 31,9 % son
frágiles y 30,6% son pacientes geriátricos complejos. Los hombres son en su mayoría
frágiles (35,1%) y las mujeres son pacientes geriátricas complejas (31,9%); existe
asociación entre la valoración clínica y el sexo (p < 0,05) (Figura 1). La distribución de la
valoración clínica fue similar en el grupo de 60 a 79 años, en el grupo mayor de 80 años
aumentaron los casos complejos (Figura 2, p<0,05).
Las enfermedades crónicas más frecuentes en la muestra de estudio fueron
hipertensión arterial (42,5%), artrosis (33,6%), dislipidemias (29,6%), enfermedades
cardiovasculares (17,9%) y diabetes mellitus (14,6%).

VALORACIÓN FUNCIONAL
La mayoría (79,4%) de los adultos mayores que acuden a los establecimientos de salud
de la Disa Callao son independientes, es decir si realizan las actividades básicas de la
vida como lavarse, vestirse, uso del SSHH, movilizarse, continencia y alimentación sin
depender de alguien, 10% son dependientes parciales y 10,6% son dependientes
totales.
No se encontró relación entre el sexo y la valoración funcional; sin embargo, a mayor
edad se encontró un aumento del nivel de dependencia (p<0,05).

Discusión
La mayoría de estudios sobre Valoración
Geriátrica Integral en nuestro país han
sido realizados en pacientes
hospitalizados, por lo que el principal
objetivo del estudio fue investigar las
enfermedades prevalentes, valoración
funcional valoración socio-familiar del
Adulto mayor que acude para atención a
los establecimientos de salud del primer
nivel de atención de la Disa Callao. Estos
resultados podrían reflejar lo que ocurre
en otros lugares del país con
características socio-demográficas
similares.

Varela (2004) define la valoración
geriátrica integral (VGI) como "un
proceso diagnóstico evolutivo
multidimensional e interdisciplinario,
diseñado para identificar y cuantificar los
problemas físicos, funcionales, psíquicos
y sociales que puedan presentar los
adultos mayores, especialmente los más
frágiles, con el propósito de desarrollar
un plan individualizado de cuidados
integrales, que permita una acción
preventiva, terapéutica, rehabilitadora y
de seguimiento, con la óptima utilización
de recursos a fin de lograr el mayor
grado de autonomía. Refiere además que
la Valoración Geriátrica Integral debe
implementarse en el Perú, tanto en la
atención primaria de salud como en los
hospitales y fuera de ellos. (7)
Encontramos que las mujeres
consultaron más que los hombres
(69.77% para las mujeres y 30.23% para
los hombres). Los que acuden con
mayor frecuencia son los que tienen
entre 60 y 69 años de edad (44.5%),
resultado similar al encontrado por
Lisigurski en una población de Barranca
(8).
En relación a la Valoración funcional
(Escala de Katz), ésta demostró índices
de funcionalidad aceptables en las
actividades básicas en 79.40%, siendo
menor aquellos con algún grado de
dependencia (20.60%). Estas cifras
coinciden con lo hallado por DIAZ T, en
su investigación “Evaluación geriátrica
en la atención primaria” en las que
utilizando diferentes escalas de
funcionalidad mostró que un 87% son
independientes. (9)
Un estudio realizado por Rubén Suárez y
Claudia Pescetto (10) menciona que la
mayoría de los sistemas de seguridad
social de la Región de las Américas
funcionan mediante mecanismos de
reparto, cuyo financiamiento proviene
de las contribuciones que realizan los
empleados. Las pensiones otorgadas se
definen de acuerdo con la remuneración
a la edad del retiro y el período de
tiempo durante el cual el empleado
contribuyó financieramente. Solo en
Argentina y Canadá se aplica un sistema
de seguridad social universal con
programas que garantizan una pensión
mínima a toda la población o a grandes
segmentos de ella. En Argentina, este
componente universal que forma parte
de la pensión se denomina prestación
básica universal (PBU) y se reajusta
anualmente. En Canadá, el programa de
seguridad para adultos mayores (old age
security program, OASP) garantiza a la
población un subsidio o pensión única
que se otorga a todas las personas cuyos
ingresos anuales sean inferiores a US$ 35
000. El OASP funciona a la par del
sistema contributivo de reparto que es
parte de la seguridad social (obligatoria).
En el caso de Ecuador, además de las
pensiones basadas en el reparto, se
otorga una pensión mínima o reembolso
a las personas cuyos ingresos o
contribuciones sean insuficientes para
ingresar al programa de pensiones.
En Venezuela se otorga una pensión
básica fija complementada por una suma
que depende de los ingresos y una
subvención adicional equivalente a 10%

de los ingresos del beneficiario, todo
como parte de un mismo sistema.
En el Perú las remuneraciones son bajas. En el año 2002 el 41.66% de la población adulta mayor estaba afiliada a un sistema de pensiones; de este el 97% al Sistema Nacional de Pensiones (SNP) y el resto a un seguro privado de pensiones. (11) En nuestro estudio se encontró que un
61.46% de los adultos mayores percibía
alguna pensión mientras que un 38,5%
no reciben pensión ni otros ingresos y
46,2% no cuenta con seguro social. Un
3,1% se encuentran en situación de
abandono familiar,
En relación al seguro de salud encontramos que el 66.11 % no cuenta con un seguro de salud similar a lo reportado en el estudio Perfil del Adulto Mayor PERÚ – INTRA II 2004 .Desarrollando respuestas integradas de los Sistemas de Cuidados de la Salud para una población en rápido envejecimiento INTRA II – PERÚ (10) reportaba que solamente el 31.4% de la población adulta mayor estaba afiliada a ESSALUD; un 7.2 % a Seguros Privados, mientras que el 61.4% no se hallaba afiliado a ningún sistema. En nuestro estudio se halló que sólo
24,9% tiene una valoración socio-familiar
de buena o aceptable, similar a lo
encontrado por Lisigurski (8) que halló
que un 86.6 % de los adultos mayores
que habitan en Barranca presentan
riesgo y problema social. Encontramos
además que el sexo esta asociado al nivel
de valoración socio-familiar (p<0,05), los
varones tienen una mejor situación que
las mujeres (29,7 vs. 22,9%), 50,9% de las
mujeres se encuentran riesgo social.
Del total de Adultos mayores que acuden
a los establecimientos de salud de la Disa
Callao el 13.62% son categorizados como
saludables es decir que no manifestaron
tener antecedentes de riesgos, el 23.92%
son enfermos presentan alguna afección
aguda, sub.-aguda o crónica, el 31.89%
es frágil ya que manifestaron tener
alguna enfermedad crónica y el 30.56%
es complejo con tres o más
enfermedades crónicas, lo que es
concordante con lo señalado en la
literatura. Valderrama-Gama (12),
encontró que aun en una muestra
probabilística poblacional de 1.001
sujetos, sólo 4,5% no declaraban
patologías.
La Hipertensión arterial, Enfermedad
Cardiovascular, Diabetes mellitus,
Tuberculosis fueron las patologías de
mayor prevalencia, lo que está en
concordancia con la mayor frecuencia de
estas enfermedades como causa de
defunciones (13). El grupo de
enfermedades del sistema circulatorio
alcanzó al 16.8% todas las muertes.
CONCLUSIONES
1. La edad promedio de los Adultos mayores que acuden a los Establecimientos de Salud de la DISA I CALLAO es de 71 años.
2. El 69.77% de los adultos mayores que acuden a los establecimientos de salud es del sexo femenino y el 30.23% .del sexo masculino
3. El 58.14% tiene Primaria como grado de instrucción

4. El estado civil predominante es el casado con 39.53%, seguido del viudo con .32.56%.
5. El 5616% es ama de casa siendo esta la ocupación más frecuente.
6. El 66.11% no cuenta con un Seguro de Salud.
7. Según la valoración clínica el 31.89% es frágil seguido de complejo con 30.56%.
8. Según la valoración funcional el 79.40% es independiente, seguido de dependiente total con 10.63%
9. Según la valoración socio-familiar el 46.18% vive en Riesgo Social, el 28.90% vive con Problemas sociales.
10. En relación con la prevalencia de
enfermedades de carácter crónico
se encontró que las más frecuentes
son: Hipertensión Arterial, 26.23%;
Osteoartrosis, 20.7%;
Dislipidemias, 18,24%; Enfermedad
cardiovascular (11,07%) y Diabetes
Mellitus (9.02%).
RECOMENDACIONES
1. Facilitar el acceso a la atención de salud a los Adultos Mayores que se encuentran en riesgo social, a través de redes sociales de apoyo.
2. Elaborar guías de manejo para las enfermedades prevalentes en el Adulto Mayor con el fin de unificar criterios y mejorar la calidad de atención.
3. Fortalecer las capacidades de los recursos humanos para la atención integral del adulto mayor en el primer nivel de atención.
4. Promover la adopción de Estilos de vida saludables en el Adulto Mayor.
5. Educar a la población de adultos mayores y a sus familiares cercanos, en la prevención de accidentes domésticos, como las caídas, que son una fuente importante de discapacidad y muerte.
6. Diseñar un Plan de Salud Preventivo del Adulto Mayor que se realice en la atención primaria de salud, para identificar los factores de riesgo para las enfermedades prevalentes en ese grupo de edad y abordarlos multidisciplinariamente, a su vez detectar precozmente la aparición de dolencias crónicas, de manera que su manejo oportuno evite la aparición de complicaciones posteriores.

Era una dulce niña, en este
mundo de duelo y de aflicción.
Miraba como el alba pura;
sonreía como una flor.
Era su cabellera obscura
hecha de noche y de dolor.
Yo era tímido como un niño. Ella, naturalmente, fue,
para mi amor hecho de armiño,
Herodías y Salomé...

Y más consoladora y más
halagadora y expresiva, la otra fue más sensitiva

YO PERSIGO UNA FORMA
Yo persigo una forma que no encuentra mi
estilo,
botón de pensamiento que busca ser la rosa;
se anuncia con un beso que en mis labios se
posa
al abrazo imposible de la Venus de Milo.
Adornan verdes palmas el blanco peristilo;
los astros me han predicho la visión de la
Diosa;
y en mi alma reposa la luz como reposa
el ave de la luna sobre un lago tranquilo.
Y no hallo sino la palabra que huye,
la iniciación melódica que de la flauta fluye
y la barca del sueño que en el espacio boga;
y bajo la ventana de mi Bella-Durmiente,
el sollozo continuo del chorro de la fuente
y el cuello del gran cisne blanco que me
interroga.

Semblanza: Atos Alves de Sousa
No dia 17 de novembro de 2013, às 23
horas e 45 minutos, parou de funcionar o
cérebro que de forma genial restabelecia a
função de cérebros. Morreu Atos Alves de
Sousa, o maior neurocirurgião de Minas e um
dos expoentes máximos da neurocirurgia
brasileira e mundial.
Atos graduou-se em medicina em 1972.
Após especialização em universidades
europeias, iniciou vertiginosa atividade
neurocirúrgica na Santa Casa de Belo Horizonte.
Transformou esse hospital em um dos principais
centros de excelência da neurocirurgia
brasileira. Rapidamente, seu nome ultrapassou
as fronteiras do país e ele tornou-se
neurocirurgião do mundo, com estadias
frequentes em vários países de todos os
continentes para aulas e cirurgias.
Três particularidades definiram Atos
como grande médico, um gênio da
neurocirurgia: o amor pelo ser humano, a paixão
pelo trabalho e a persistência na aquisição da
técnica apurada.
Atos era amoroso, afetuoso, generoso,
acolhedor, protetor, amigo. A essência da
medicina é o amor pelo ser humano. Quando a
maestria de sua técnica chegava ao limite, Atos
a expandia com a atenção e o conforto a seus
pacientes, que o amavam intensamente. A cena
mais chocante em seu velório, ao mesmo tempo
triste e bela, foi o grupo de uma dezena de
pacientes, por ele operados, em círculo em torno
de seu corpo, chorando o fim da vida daquele
que lhes tinha salvo a vida.
Sem paixão não se faz nada de grande
na vida. Com paixão, Atos levou seu ofício às
raias da perfeição, transformando-o em arte. E
sua felicidade era encontrar nessa arte o prazer.
Era esta paixão que fazia este virtuoso
do bisturi perseguir a perfeição do gesto
simples, elegante, preciso. Sua mão
gentil, de artista chinês em porcelana da era
Ming, tocava o cérebro como um sacerdote toca
um ícone sagrado.
Com muita arte, Atos reparava o
cérebro, a matéria mais elaborada do universo,
o templo de todas as artes. Sua cirurgia não era
guerra contra a doença, mas delicada interação
com a lesão. E esta saía quase pacificamente,
com pouco derramamento de sangue.
Nossa arte reflete nossa vida, pois
nada pode sair do artista que não esteja no
homem. A beleza da arte cirúrgica de Atos era
simplesmente reflexo de sua bela mente.
É comum, a genialidade se associar ao
gênio irascível e orgulhoso. Atos, apesar do
reconhecimento mundial como mestre e ícone
da neurocirurgia, era comedido, modesto e
humilde. Ou seja, alcançou a sabedoria ao
associar a fina técnica à arte de bem viver. Este
paradoxo do profissional genial na pele do
homem simples e afetuoso encantou seus pares
nos quatro cantos do mundo. Atos partiu
carregado de amigos e admiradores.
Afirma-se que homem algum é
insubstituível. Mas, como substituir um
Miquelângelo que esculpia vida no mármore?
Como substituir um Atos que com técnica
apurada esculpia o cérebro para restituir-lhe
forma e função? Não se substitui Miquelângelo
ou Atos e nem é necessário, pois eles vivem em
suas obras e naqueles que se inspiram em suas
artes. Atos continuará vivo, iluminando a
neurocirurgia brasileira.
O importante não é o nascimento ou a
morte, mas a travessia pela vida. O que nos
resta de Atos não são suas cinzas, mas a
chama de sua vida vitoriosa.

mostra o significado da imortalidade para

aqueles que tiveram vida útil e frutífera, quando
o criador afirma: farei de vossos túmulos berço
de vidas. Vidas continuarão a ser salvas através
dos ensinamentos de Atos e das mãos de seus
discípulos.
O que nos choca é ver majestosa
árvore carregada de frutos, e fadada a várias
florações, ser decepada abruptamente. Atos
saiu de nossas vidas de forma inesperada.
Como num encantamento. Parece que ele
continua encantado entre nós. E continua como
sempre foi, encantador.
Querido Atos,
Tua vida se resumiu em levar vida a
teus pacientes e alegria a teus familiares e
amigos. Te perdoamos por hoje nos fazer
chorar. Mas não choramos por ti. Choramos por
nós mesmos, pela falta que tu nos faz. Mas a
dor de tua ausência nunca será maior do que a
alegria de tua presença entre nós.
Tua morte é tuas núpcias com a
eternidade; apenas uma mudança de palco.
Continuarás vivendo nos cérebros daqueles que
te amam e nos cérebros que tuas mãos
salvaram.

Reglamento de publicaciones
a Revista NEUROCIRUGIA / NEUROCIRURGIA es el órgano oficial de difusión científica de la
Federación Latinoamericana de Sociedades de Neurocirugía (FLANC), dedicada a la
publicación de artículos de neurocirugía y disciplinas afines. Es una revista cuyos artículos
pueden ser escritos sea en el idioma español o portugués, con publicación semestral. Los
objetivos son difundir formalmente las experiencias de los neurocirujanos así como los
avances que se produzcan en el diagnóstico y tratamiento de las diversas patologías neuroquirúrgicas; e
intercambiar el conocimiento científico con entidades similares del extranjero promoviendo la
investigación y avance de la Neurocirugía en forma integral, globalizada y multidisciplinaria conforme
en esencia que representa neurociencias. La finalidad es contribuir en la mejora del pronóstico y
calidad de vida de los pacientes.
NORMAS GENERALES Tratar sobre temas de Neurocirugía u otras
áreas de Neurociencias Los artículos deben ser originales e inéditos Pertenecer a una de las siguientes
categorías: - Editorial. - Página del Presidente - Artículos Originales. - Artículos de Revisión. - Reporte de Casos. - Notas Técnicas. - Miscelánea neurociencias - Galería Fotográfica. - Cartas al Editor.
Estar redactado en español o inglés, impreso en papel bond blanco A4, espacio simple, con márgenes de 3 cm.
Cada sección del manuscrito empezará en página aparte, las que se numerarán en forma consecutiva.
Se entregará 1 original impreso y la versión electrónica del texto grabado en CD o USB, en el programa Word para Windows 97/2000 o XP y los gráficos en MS-Excel, las imágenes y mapas deben ser grabados en formato TIFF a una resolución mayor de 500 dpi. De preferencia fotografías con buena resolución.
Se consideran figuras a los dibujos, mapas, fotografías o gráficos ordenados con números arábigos. Las leyendas deben ser escritas en una hoja aparte; las leyendas de microfotografías deberán indicar también el aumento y el método de coloración. Los mapas también deben tener una escala. El Comité Editorial de la revista se reserva el derecho de limitar el número de ilustraciones.
En la primera página del original se consignará:- Identificación del autor o autores en el siguiente orden: nombre, apellido paterno e inicial del apellido
materno, filiación institucional, ciudad y país. Nombre de la institución o instituciones en las que se realizó el trabajo. Nombre y dirección del autor responsable de la correspondencia, apartado postal, teléfono y correo electrónico.
El título del artículo debe ser corto y claro en castellano o portugués. Puede tener subtítulo corto referido al tema principal del estudio.
Las referencias bibliográficas serán única-mente las que han sido citadas en el texto, se ordenarán correlativamente según su aparición y se redactará siguiendo las normas del Uniform Requirements for Manuscripts Submitted to Biomedical Journals del Comité Internacional de Editores de Revistas Médicas, en su versión actualizada.

Introducción: Exposición breve de la situación actual del problema y objetivo del trabajo o hipótesis.
Materiales y Métodos: Describir las características y selección de la muestra y la metodología utilizada en el estudio. Evitar describir en detalle los procedimientos conocidos.
Resultados: Presentación de los hallazgos, en forma clara, sin opiniones ni interpretaciones, salvo en las de alcance estadístico. Se pueden complementar con tablas, figuras o fotografías.
Discusión: Interpretación de los resultados, comparándolos con los hallazgos de otros autores, exponiendo las sugerencias, postulados o conclusiones a las que llegue el autor.
Conclusiones: Síntesis de los hallazgos y aportes más importantes del artículo.
Referencias bibliográficas: De acuerdo con las normas del Uniform Requirements Manuscripts Submitted to Biomedical Journals del Comité Internacional de Editores de Revistas Médicas, en su versión actualizada.
