130 Pctes con Enfemedad DA proximal: objetivo clínico primario: libre de muerte, IAM o nueva RVM. A 1 año: PCI no inferior a MIDCAB (Evento primario 7.7 % en ambos casos). Nueva RVM: PCI 6.2% vs 0% MIDCAB. IAM: MIDCAB 7.7 vs 1.5 % PCI.
●HANNAN EL (2014): Seguimiento a 3 años Registro clínico de NY: CABG vs DES en 715 pacientes pareados, con lesion a DA proximal. Sin diferencias significativas en mortalidad a 3 años (Hazzard Ratio 1.14, 95% CI 0.70-1.85) o
Mortalidad, IAM-EVC (HR 1.15, 95% CI 0.76-1.73). Tasa de nueva RVM menor en CABG (HR 0.54, 95% CI 0.36-0.81).

Revascularization Recomienda revascularizacion para enfermedad en DA significativa (lesion >50%
+ isquemia, FFR <0.80). Recomienda CABG en TCI o multivaso con SYNTAX >22 si el riesgo quirúrgico es
bajo.

PCI Balón VS CABG: Multivaso Inicialmente pequeños estudios aleatorizados: CABG = MENOR mortalidad a 5
años (10.2 vs 15.8%) y 10 años (26.4 vs 30.5%).
Desarrollo de Estudios aleatorizados demostró no diferencias en mortalidad, pero si ventaja de CABG en términos de: Nueva RVM.
BARI (Bypass Angioplasty Revascularization Investigation): Efectividad de ambas técnicas en 1829 pctes (2-3 vaso), anatomía similar, resultados
a 5, 7 y 10 años.

NUEVA REVASCULARIZACION MENOS PROBABLE EN CABG vs PCI: 8 vs 54%, 13 vs 60% y 23 vs 77%.
ANGOR menos frecuente en CABG en 1er año, sin diferencia lo 10 años (16% ambos grupos).

RITA (Randomized Intervention Treatment of Angina): 1011 Pts. Seguimiento promedio 6.5 años. 55% multivascular.
MORTALIDAD SIMILAR (CABG-PCI: 17 vs 16%).
ANGOR más frecuente en grupo Angioplastia (26% ameritó CABG y 19% nueva angioplstia).

practica actual en países desarrollados.
ARTS I : 1205 pacientes (17% DM2). Aleatorizados a BMS o CABG.
Enfermedad de 2 (70%) y 3 (30%) vasos. Revas completa mas posible en CABG (84% VS 71%).
A 5 años: CABG-BMS MORTALIDAD similar (8 vs 7.6%). MUERTE-IAM-EVC similar ( 18.2 vs 14.9%).

Pacientes elegibles para cualquier procedimiento.
Se desarrolla score SYNTAX asignado a cada paciente, evaluando complejidad anatómica de lesiones.
3 grupos en orden creciente de complejidad : 0-22, 23-32 y >33

RESULTADOS : MORTALIDAD, EVC, IAM, RVM a 12 meses (significativamente
mayor en PCI (17.8 vs 12.4%). EVC significativante mayor en CABG (2.2 vs 0.6%).
RVM completa más probable con CABG (63 vs 57%).

Paciente con adecuada información sobre opciones de manejo.
CONCLUSIONESPCI recomendada en: 1 Vaso sin DA proximal Riesgo quirúrgico alto y antomía favorable.
PCI similar a CABG en: 1 vaso, DA proximal afectada 3 vasos SYNTAX bajo
CABG preferida en: SYNTAX medio o alto 2-3 vasos con o sin DA proximal.
